| Q1 | Would you please have another Mandatory site visit later this week or early next week? |
| A1 | Another site visit was scheduled for 7/2/2018 at 10:00am. |
| Q2 | As per conversation with the Bidders on the Mandatory Site Visit, June 26, 2018, for clarification, Government Furnished Equipment/Supplies to the Contractor? |
| A2 | As per Performance Work Statement (PWS), yes. |
| Q3 | Is there a building 322? |
| A3 | Yes, it is the CT scan area. It is patient care. |
| Q4 | Can we have the breakdown of carpet and VCT flooring in building 330? |
| A4 | Ratio is 60%/40%. 60% hard and 40% carpeted. |
| Q5 | What is the budget for this project? |
| A5 | A budget is not set, the competition from the Solicitation will help determine the budget as |
this is the first time for this requirement.
| Q6 | How many EMS staff members are covering these locations? |
| A6 | There is 2nd shift staff that cover these areas. 18 housekeepers in the shift. |
| Q7 | Can we have a list of room numbers that we are responsible for in building 330? |

| Q8 | Is this project intended for only local vendors? |
| A8 | There is no restriction. |
| Q9 | We would like to confirm that a ‘capabilities statement’ is to be submitted with our proposal and not a narrative of our ‘technical capabilities’ |

| Q10 | Can you provide a breakdown of SF by carpet and hard flooring requiring strip and waxing? |
| A10 | Please refer to Q4. |
| Q11 | Are we responsible for the badging cost? If so, how much is it per associate? |
| A11 | A Non-PIV (Physical Access Only) Card is required. There is no cost of badge itself, the Government covers the badge cost. However, there is a cost for fingerprinting, this cost is $25-$35 per person. This cost is to the Contractor. Additionally, Keys will be provided if there are spaces that need to be accessed. According to policy, a lost key will set a charge of a minimum $15 up to a maximum of $2000 per incident. Additionally, Keys may be issued by Service Chiefs to non-VA employees working for them in a consultant/liaison capacity, e.g., an individual working at the Dayton VA Medical Center but salaried by another medical institution. Such issuances may be accomplished only when the Service Chief makes arrangements to collect and retain from the consultant/liaison $50.00 per key issued until such time that the key(s) are returned, at which time the collected monies will be returned to the consultant/liaison. |
Q12 Ref: SOW Pg 5 Item 7 (c) “total supplies and equipment costs” Comment: the SOW states on page 9 item C that the government is to furnish equipment and “any other supplies needed to accomplish daily tasks”.

Q13 Ref: Pg 8, number 5 Frequency of Services Required:

| A13 | Chief of EMS or his designee or the COR shall make this determination based on industry |
| recommendations or findings from QA/QC or EOC rounds. |

| Q17 | Question: Will the government please provide a detailed breakdown of flooring types by SF, per building. i.e. Bldg 999 - 20,000 SF of carpet, 30,500 SF VTC, 10,000 SF Laminate. |
| A17 | Ratio is 60%/40%. 60% hard and 40% carpeted. |

| A18 | Approximate number of restrooms to be serviced is 150. |
| Q19 | Question: How many windows are to be cleaned? |
| A19 | Interior of windows were to be cleaned only. No exterior window cleaning is required. Windows will be cleaned with supplied cleaning gear by EMS, i.e. glass cleaner, squeegee. There are approximately 2,100 windows included in the 17 buildings. However, contractor must remember this number includes patient rooms that are NOT to be cleaned by the contractor. |
| Q20 | Question: On the site visit it was mentioned that the contractor was not to dust desks, desk chairs, computers, bookshelves and other like personal work space items. Will the government please confirm this is SOW’s intent? |

| Q25 | It was stated at the site visit that the contractor is only responsible for labor only and none of the following; cleaning equipment, supplies, chemicals, etc. Can you confirm this please? |
| A25 | This was addressed during the walk through and within the PWS. |
| Q26 | It was stated we could request and receive a schematic drawling of the buildings requiring housekeeping services with the correct square footage and with rooms marked do not clean or possibly shaded out. Can you provide this? |
| A26 | A schematic of the buildings could be provided; however, the plans would not be marked with each room that needs to be cleaned or square footage. A request has been put in with Engineering to provide drawings, EMS is still waiting on receipt of drawings. |
| Q27 | The square footage provided seems to be incorrect. Can the correct cleanable square footages be provided for all 17 buildings? Several areas were either larger or smaller. |
| A27 | Please use the square footage provided in the Performance Work Statement (PWS) and Amendment 0001 to the Combined Solicitation/Synopsis. |
| Q28 | It was stated that some areas require high dusting which requires lifts or very tall ladders that are provided by the VA. |
| A28 | Yes, this is correct. |
| Q29 | It was stated that an office would be provided for the contractor to work out of and for employees to clock in & out at. Is this correct? |
| A29 | Office will be provided. It is the contractor’s responsibility to clock in and out their employees. |
| Q30 | Can you provide the number of stairwell in building 330 the VA Hospital? |
| A31 | Seven stairwells |
| Q31 | Does the contractor work on holidays? |
| A31 | This is addressed in the PWS. |
| Q32 | The VA Medical Center building 330 is asking for very high dusting which requires the use of very tall ladders. This is not in the scope of work. Please advise. |
| A32 | This was addressed during the walk through. Also, refer to Q38/A38. |
| Q33 | Can you provide a map of the entire campus? |
| A33 | Yes. |
| Q34 | Can you provide a floor plan map of each building? |
| A34 | These have been requested from engineering, still waiting to receive. |
| Q35 | Can you provide an estimated square footage of carpeted areas vs. hard floor areas for each building? |
| A35 | Refer to Q4/A4. |
| Q36 | During the walk through, there were areas of multiple buildings that we were told we would not be responsible for cleaning them. Can we get an estimated square footage or percentage for each building for the areas we ARE responsible for? |
| A36 | Refer to Q4/A4. |
| Q37 | Can you provide us with an estimated square footage for any storage space or workspace that will be provided to us (the contractor) for our equipment storage or potential office workspace? |
| A37 | There is not a set work space at this time for office workspace. EMS has a warehouse that can be used to store any contractor equipment needed. |
| Q38 | During the walk through it was mentioned we would be able to use ladders or possibly lifts provided by the VA when doing the high cleaning requirements in the lobby of building 330. Can we get confirmation that this is true? If yes, what types of equipment would be available to us? i.e. Height of ladder, scissor lift or boom lift, etc. |
| A38 | Yes, this is true. Scissors lift, ladders will be provided. |
| Q39 | Can we receive a list of any chemicals and materials that will be provided by the VA for contractor use? Would any of the chemicals include spray bottles for use or come in aerosol form? |
| A39 | Yes. No aerosol. |
| Q40 | Does the current crew use the chemical method for stripping wax from the floors? If yes, are we expected to also use this method? We prefer the chem-free machine method for stripping floors as it is faster, easier to keep clean, and more environmentally friendly, would we be allowed to use this method? |
| A40 | Contractor can use any method that complies with VA and EMS policy. |
| Q41 | During the walk through we were told as a contractor we will be expected to provide labor and specialty equipment and we would be able to use regular machinery and equipment provided by the VA. Can we receive confirmation that this is correct, if so can we receive a list of any machinery and equipment we would be able to use? i.e. vacuums, janitorial carts, large trash carts, floor buffers, mopping systems, auto-scrubbers, cleaning rags, etc. |
| A41 | DVAMC shall supply all supplies and equipment needed to perform cleaning services. |
| Q42 | Is there a limit to the number of keys we can be assigned? This would help with potential staffing numbers for managers/leads. |
| A42 | DVAMC protocol will be followed. |
| Q43 | How many exterior windows are there to be cleaned semi-annually? |
| A43 | Not part of this solicitation. |
| Q44 | Can the exterior windows be cleaned by ladder? |
| A44 | Not part of this solicitation. |
| Q45 | Is there terminal cleaning involved? |
| A45 | No, no patient care areas to be cleaned by contractor. |
| Q46 | In the solicitation it states the following: |
GOVERNMENT FURNISHED EQUIPMENT/SUPPLIES
(paper towels, toilet paper, hand soap, hand sanitizer, detergent, disinfectant) solution, power equipment and any other supplies needed to accomplish daily tasks.
But right under that it states the following:
1. CONTRACTOR FURNISHED EQUIPMENT/SUPPLIES
1. The contractor shall be furnished the supplies necessary to perform all services required. Supplies and equipment shall meet the specifications below and comply with VA, federal or state Occupational Safety and Health Standards and fire regulations. Contractor furnished items shall be subject to use by VA employees. A listing of these items follows, paper towels, toilet paper, hand soap, hand sanitizer, power equipment etc.).
Can you clarify if the Government or the Contractor will supply the cleaning items?
A46 Government provided. Yes, both items state the contractor SHALL be furnished the supplies necessary to perform all services required. If contractor brings in their own items/equipment it must conform to these specifications.
| Q47 | Do you anticipate an extension by amendment to the subject solicitation? |
| A47 | Yes, it will be extended to COB (1600 hours) Friday, July 13, 2018 |
| Q48 | Does the statement "remove work to be completed annually" in the last amendment mean that just annual cleaning or does this also refer to semi-annual as well? |
| A48 | There was an Annual column in the table that was deleted. The one item within this column was moved over to the semi-annual column. Essentially, when contract is in the 5th month, they will complete these requirements. |
| Q49 | The statement of work looked the same. Will there be a revised statement of work and price schedule? |
| A49 | The Performance Work Statement was updated to remove one-year base, the four option years, annual column of work to be completed, etc. Yes, a new price sheet is included with this amendment. |
| Q50 | Is this requirement know longer needed beyond six months? Why have the option years been removed? |
| A50 | No one is allowed to replace Government workers without a Competitive Sourcing effort, which is currently not approved or appropriated by Congress. We can augment for a temporary time frame. Option years were removed due to augmenting for a temporary time frame. |
| Q51 | This project is going to require a large staff that will be unemployed in 6 months. Is there a possibility of an extension beyond 6 months? |
| A51 | Amendment 0004 to Combined Solicitation/Synopsis added FAR Clause 52.217-8, Option to Extend Services. However, there is no guarantee this will occur. |
