About this file
This is a Performance Questionnaire template designed to collect past performance information on Offerors (Landlords/Lessors) competing for award of a Department of Veterans Affairs lease opportunity (RLP # 36C24W25R0115).
The questionnaire consists of three sections. Section 1 requires the Offeror to provide company name, phone number, and email address, with an option to indicate if the company has no relevant past performance. If this box is checked, no further completion is required. Section 2 requests evaluator information from current or former tenants, including name, title, contact details, lease square footage, organization/company name, and use of space designation (Office, Medical, or Other). Section 3 contains eight past performance rating questions covering: pricing of change orders during tenant improvement buildouts; ability to stay on schedule during buildouts; ability to stay within budget during buildouts; response to maintenance issues; response to safety issues; response to security issues; ability to keep building premises and common areas clean; and overall performance during the lease term. Each question uses a standardized rating scale (Excellent, Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) with space for written comments explaining how scores could have been improved. The Lease Contracting Officer is Alexander Ortiz, Department of Veterans Affairs, RPOW, reachable at alexander.ortiz@va.gov or 559.241-6447. Responses are treated as confidential and procurement sensitive, with handwritten responses accepted.
