Re: Panacea – VA Louisville PM Work Qualus Job Number: 851860
Rick;
Attached is the report on Panacea – VA Louisville PM Work that was performed at 800 Zorn Ave. Louisville, KY 40206.
In conjunction with the execution of the project, we have provided the attached report documentation. During this preventative maintenance service, multiple arc flash values on the existing arc flash labels were found to be noticeably out of range from typical NFPA values for transformers and main breakers. A full power study is recommended to verify accuracy of these labels and update as needed. Breakers in the 13.8kV switchgear were consistently failing on insulation resistance. This is due to the age of the breakers. Switchgear replacement is recommended. Some transformers show signs of deterioration. Recommend yearly testing to start trending data. Transformer #1 primary cables are not properly terminated to seal entrance from moisture. There should be a compression pin. Transformer #1C, Transformer #2, Transformer #4 and Transformer #19 are low on oil. Transformer #4 primary bushing wells need end caps. The red dust caps do not provide protection from flash over. Transformer #19 was not tested due to inaccessible secondary. Primary bushings showed evidence of leaking. Relay protecting Hillcrest Main Bldg 1 Phase B has an output wire lifted on the back of the relay. This should be further investigated to ensure proper breaker operation in the event of a fault. Breakers in Sub 1D could not be tested via secondary injection. The test set required is no longer available for purchase or rental. These breakers should be replaced with updated unit and perform a coordination study to determine settings for the new trip unit. Circuit 6/10 Selector Switch, T-19 Primary Switch, PNL 3XA and PNL 4XB were not tested at time of service. Equipment was continuously energized by either utility or generator. Recommend scheduling a complete outage on this equipment for electrical testing. Breakers for BLDG 6 FDR 3, BLDG 6 FDR 4 and BLDG 7 PNL 2 tested out of tolerance on contact resistance. Recommend replacement for these units.
Please refer to the individual test result sheets for additional details.
If you have any questions, or if we can be of further assistance, please feel free to call our office.
Thank you for the opportunity to perform this service.

PROJECT SUMMARY
Tri‐annual year gear maintenance at Louisville KY VA Medical Center.
SCOPE OF EQUIPMENT
Qualus Services shall cover the equipment as provided by VA Medical on their 1‐line diagrams and provided equipment counts
TESTING SCOPE OF WORK
Transformers:
1. Transformer insulation liquids shall be sampled and tested per the below requirements. If oil samples are found to be contaminated, Qualus Services will provide a new proposal to ensure contaminants are evacuated from the system by either replacing all of the fluid or by vacuuming the system.
If a transformer has been determined to be gassing or low on oil, Qualus will either add oil, replace all of the oil, or process the oil. The solution depends on if the transformer is filled with mineral oil, FR3 or silicone oil and the amount of oil in the transformer. Any of these solutions will require significant downtime on the transformer in order to remedy (24 to
72hours). Pricing for each solution will depend on the gallons of fluid are in each transformer. Our budgetary estimate would be $35,000 per transformer for a transformer up to (500 gallons) of oil.
a. All transformers listed shall be inspected. Cables, connections, and switches will be inspected and cleaned. Infrared the transformer fins for indications of blockage.
b. Oil from transformers shall be sampled in accordance with ASTM procedures and analyzed by an independent laboratory for:
i.Color ii.Acidity iii.Interfacial tension iv.Dielectric strength v.Dissolved gas‐in‐oil vi.Power factor
Also analyzed oil for PCB contaminants and determine parts per million content in accordance with either ASTM or EPA procedures.
Test insulation resistance, power factor, and turns ratio.

e. Replace gaskets as needed
f. Qualus Services is taking exception to this request. If this service is required
Qualus Services will provide a follow up proposal to complete the task based on what was found during the outage. Also, this type of repair may require significant downtime, oil processing or even having the transformer sent to a factory for repair.
g. Liquid filled transformers shall have the cooling liquid checked and replaced when test indicates that it no longer meets manufacturers’ requirement.
h. Qualus Services is taking exception to this request. If this service is required Qualus Services will provide a follow up proposal to complete the task based on what was found during the outage.
