Seed Project # 614-19-112 Install OR Isolated Power Systems

1. Reference solicitation pages 16 & 23 of 117, paragraphs 5.1 and 5.2. In both locations, we are told to submit “All Volume [I & II] proposals … in a single three-ring binder.”

As we are to provide four hard copies of each volume (original +3 copies, plus CD), we would like to confirm that you are expecting to receive only two three-ring binders for the entire submission. I.e., One three-ring binder with four copies (plus CD) of the Technical proposal and one three-ring binder with four copies (plus CD) of the Price proposal. Can you confirm this intent?

Answer: Offerors are to submit four (4) separate hard copies (1 original + 3 copies), each in its own binder, for each Volume. That’s a total four (4) separate binders for each Volume.

2. Reference solicitation page 15 of 117, paragraph 3.1: About halfway down that paragraph, it states: “Technical and Past Performance Volumes shall not exceed the page limits stated herein.” We believe this should be “The Technical and Past Performance sections of Volume I shall not exceed the page limits stated herein.” If you agree, then the word “Volume” in the following two paragraphs should also be deleted.

Answer: Yes, it should read “Technical and Past Performance “sections” of Volume I shall not exceed the page limits state herein”. These are sections of Volume I and not the individual Volumes themselves.

3. Reference solicitation page 16 of 117, paragraph 5.1., Tab (B) Factor 1 – Past Performance, subparagraph (1): This references “Past Performance Relevancy Questionnaire (Attachment A).” We believe this should be “Past Performance Questionnaire (Attachment 2).” Do you concur?

Answer: No, this section is referencing the attachment named “S02

ATTACHMENT A PERFORMANCE RELEVANCY SURVEY”.

4. We believe that, in all cases, “Sub-Factor 1 Experience” is in reference to the information provided on “S02 Attachment A Performance Relevancy Survey.” We further believe the requirement to provide the information at the bottom of solicitation page 20/top of page 21 is superseded by the information required per Attachment A. Do you concur?

Answer: No, provide ALL information as stated in the solicitation requirement. Do not assume certain information/documents supersedes another and that specific information is already contained in another section. Carefully provide all documents/information as the solicitation states.

5. Reference file name “S02 ATTACHMENT 2 VA PAST PERFORMANCE QUESTIONAIRE.” Internally to this document, it says “Attachment A - Past Performance Questionnaire.” We believe the name internally should read, “Attachment 2

- Past Performance Questionnaire.” Do you concur?

Answer: The attachment named “ATTACHMENT 2 VA PAST PERFORMANCE QUESTIONNAIRE” reads as it should.

6. Reference “S02 Attachment B Subcontractor Information and Consent Form” and the CLIN Schedule. Answer:

Phase I - Room BW309, Operating Room 7 and Room BW311 (Laser Panel IPL) Phase II - Room BW312A, Operating Room 10 Phase III - Room BW317, Operating Room 8 and Corridor CW 302 (After Hours) Phase IV - Room CW303, Operating Room 9 and Corridor CW302 (After Hours)

Because of the Laser Panel installation in BW311 that feeds the other ORs, OR 7 and BW311 work must happen in Phase I.

76. Can pictures be provided above the ceilings in the corridors?

77. Is there access to the existing two-story electrical room from inside the building or is it only accessible from the roof?

Answer: This electrical room is only accessible from the roof.

78. Please provide an elevation of the floor level for the existing two-story electrical room as it relates to the third floor.

Answer: See attached drawings. There are not a clean set of as-built drawings for this area. Note that 1-AS3.6 shows an inaccurate layout for the Endo/Hybrid OR.

Drawing A-1 shows the existing layout of Endo/Hybrid. Also, drawings 1-AS6.1 and

1-AS6.2 show the interstitial space above the OR suite and the 2-story electrical/mechanical room in relation to the OR suite. However, the layout of the rooms in the OR suite on the 3rd floor is not accurate in these drawings. For accurate layout of OR suite, refer to bid drawings.

79. Can HVAC drawings be provided?

Answer: Yes, after contract award and as relevant to the project.

80. How much lead time is required to schedule shutdowns?

Answer: For any shutdowns outside of the construction area, the contractor must notify the COR at least a week in advance.

81. Will ceilings in the operating rooms be required to be replaced at the end of each shift?

Answer: No. Ceilings in ORs are hard ceilings and ICRA barriers around each OR will remain in place until the end of each phase. So, ceilings inside each OR do not need to be put back together at the end of each work shift. Acoustical ceilings in in Corridor CW302 will need to be replaced at the end of each shift because this is a common area and will be used by surgical staff during the day.

82. Are ceilings in the existing space, both common area and in the operating rooms acoustical grid/tile or drywall/hard ceilings?

Answer: All ORs and adjacent supply and storage rooms have hard ceilings. The main corridor (Corridor CW302) has acoustical ceiling tiles.

83. What is the permit lead time for the associated Class IV work through the Hospital Safety officer?

Answer: ICRA assessment/permit is performed internally by the medical center Construction Safety Committee before contractor arrives on-site and will be issued to the contractor once they arrive on-site. There is no lead time on obtaining it.

84. As main electrical runs are being installed to the associated rooms, is it acceptable for runs of ceiling tile to be displaced, and then put back before the work shift is over?

Answer: Yes, if the work is inside the ICRA barriers as shown on drawing GI002.

85. Would engineers and owners accept having a conversation about running main electrical line above the roof line down into the space to avoid corridor work? From a technical standpoint, would this be possible?

Answer: See updated drawings in Solicitation Amendment issued with these RFIs.

Work in Corridor CW-C4 has been removed from the project and main electrical lines will now be run in the electrical/mechanical space on top of the OR. There is no change to the work in Corridor CW302.

86. Please confirm requirements for shoe coverings and adhesive walk off mat placement in space.

Answer: Shoe and hair coverings along with scrubs are required in common area of OR suite, i.e. before entering construction site or after exiting construction site.

Adhesive walk off mats are required inside the entrance of each construction site.

Refer to Drawing GI002 and Specification Section 01 35 26: Safety Requirements for more details on ICRA requirements.

87. Can barriers for a given space be left in place if work is not completed in a given room or common space (clarify for common space / corridor work)? The assumption is that the room or corridor would not be usable while construction activities are ongoing and would not be able to be put back in time for morning activity.

Answer: The drawings and specs define which rooms/ORs are part of each phase.

ICRA barriers for these ORs and rooms will stay in place until work is complete and the space is cleaned and ready to be given back to the surgical staff. For any work performed in the main corridor/common space (Corridor CW302), the corridor ceiling must be returned to how it was at the beginning of the shift and the area must be cleaned. No barriers blocking Corridor CW302 will be left up at the end of a night shift. The ceilings must be put back in place and the affected portion of the corridor must be cleaned before the end of a night shift. Drawing GI002 shows which barriers need to be removed at the end of each night shift.

88. It states that barriers are to be left in place until each phase is inspected and cleaned by Hospital Environmental staff - will the hospital environmental staff be responsible for cleaning the space following every construction work shift?

Answer: No, the contractor is responsible for cleaning up their space at the end of each shift and at the end of each phase. At the end of each construction phase, Hospital Environmental staff will perform a terminal clean after the contractor finishes cleaning their space before the space is given back to the surgical staff.

89. Will HEPA Filtration equipment remain running and in place during off hours?

Answer: Yes, HEPA filtration must be run at all times (24 hours/day, 7 days/week) while ICRA barriers are required, even if no active work is happening.

90. Clarify request for cut in boxes and fishing of flex conduit down walls - will old box locations be reused, or will they have to be patched with drywall? If they are not being reused, are blank plate covers acceptable?

Answer: Boxes shall be reused per general note C Sheet EE101 for Critical circuits.

Per note C sheet EE102 normal power receptacles are new locations. Care should be made to re-use boxes within walls. Where devices are removed and boxes are no longer used, walls to be patched and repaired per demolition note O, sheet EG001.

91. Are there additional lights in the given rooms that are going to be rewired? (2x4's or Can lights?) Junction boxes and surgical light are called out but no other lights in these rooms.

Answer: Existing lighting to remain. Fixtures may be removed and reconnected for above ceiling work.

92. Based on reading of the Phasing plan in the specs, it appears that the only work that has been requested to be after hours is the corridor work. Work in the actual rooms can be done during normal working hours, with the ICRA and ventilation measures in place -please confirm this is correct.

Answer: This is correct. Also, any core drilling into the ORs will need to be performed after hours due to noise and vibration issues.

93. Factor 1 – Past Performance – Would the Contracting Officer allow for CPARS to be emailed directly to the Contracting Officer by March 15, 2021 at 2PM, or to be included as an addendum that does not count against the page requirement in the final proposal, instead of the PPQs to be completed by the project Point of Contact? It is often difficult to obtain PPQs due to the busy nature of Contracting Officers, particularly for projects where they have issued a CPARS report.

Answer: Yes, if PPQ’s can’t be obtained, CPARS can be submitted.
