About this file

This is Amendment 0001 to Request for Quotation (RFQ) 36C24826Q0385, issued by the Department of Veterans Affairs Network Contracting Office 8 on April 29, 2026, with an effective date of April 15, 2026. The amendment extends the deadline for receipt of offers to May 15, 2026, and addresses vendor questions submitted during the solicitation period, which is now closed to further inquiries.

The RFQ solicits phlebotomist services (PSC Q702 Technical Medical Support) for seven outpatient clinics and one medical center within the VA system. The requirement is for five full-time equivalent (FTE) phlebotomist candidates in the base year and eight FTE candidates for option years. All phlebotomists must be compensated under Wage Determination WD#2015-5707, with required qualifications including a detailed curriculum vitae highlighting six months of phlebotomy-related experience, current BLS/CPR certification, and reference letters from previous employers or supervisors. Personnel must demonstrate flexibility to rotate across all eight locations based on operational demand. The solicitation is set aside 100% for small businesses. Offerors must submit quotes to the Contracting Officer, Fernando G. Defillo, at Fernando.defillo@va.gov, and pricing proposals must include a detailed price breakdown outlining all costs incorporated into the hourly rate.
