Statement of Work

Install Police Evidence Secured Storage - Bldg. 42 Project Number: 673-21-122

James A Haley VA Hospital Tampa, Fl.

1. GENERAL: This project is to provide the Police Service with an evidence secured storage area within Building 42, located at the JAHVH.

1.1. Description of Services Provided: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to perform this project in accordance with the SOW and specifications.

The contractor shall perform as to meet or exceed JAHVA Standards and TIL Specifications in this contract.

1.2. Execution: This SOW describes the design build project located in the Police Administration Offices on the ground floor of building 42 located on 131st St. This building is separate from the main hospital. The completed project will provide a complete and functioning secured storage area as reflected in the solicitation.

The contractor shall include in the design:

Ensure exterior walls are of brick and masonry construction. Exterior walls which are composed of wood frame and siding require an interior backing of steel security screen mesh or sheet partition. Perimeter walls shall be full height, floor to underside of slab above. Interstitial access is not allowed. This room allows for interstitial access and needs to be fitted with a cage that prohibits up and over access.

All vents, ducts, and similar openings in excess of 96 square inches (620 cm2) that enter or pass-through space shall be protected with either bars or grills. If one dimension of the duct measures less than six inches (150

mm) or duct is less than 96 square inches (620 cm2), bars are not required; however, all ducts must be treated to provide sufficient sound attenuation. If bars are used, they must be 1/2-inch (12.7 mm) diameter steel welded vertically and horizontally six (6) inches (150 mm) on center;

if grills are used, they must be of 9-gauge expanded steel.

Door Construction: Doors are of 45 mm (1-3/4 in.) solid core hardwood or hollow steel construction. Dutch or half doors are unacceptable.

Removable hinge pins on door exteriors must be retained with set pins or spot welded, preventing their removal. Hinge pins will be on the outside if the door opens outward.

The locking requirements (including access- controlled egress doors) outlined in National Fire Protection Association (NFPA) Life Safety Code standard, latest edition, 101-7.2.1.5 and 7.2.1.6 must be followed

Mechanical locking systems: Where mechanical lock systems are used, installed lock sets must allow for single motion egress. The installation of high security exit devices meeting NFPA Life Safety Code standards is appropriate.

Fire code: prohibits locks from being locked from the inside that require a key to exit, the intent is that there must be two locks, one of which must be key operated. The other lock can be key, combination or electronic (NFPA 101, 7.2.1.5.2) locks, if provided, shall not require the use of a key, a tool or special knowledge or effort for the operations from the egress side.

Steel doors will not be set into wooden frames.

Doors set in steel frames must be fitted with a mortise lock with a deadlock feature. IAW ANSI/BHMA A156.13 American National Standards for Mortise Locks.

The day lock on the main door must be automatically locking, with a minimum 19 mm (3/4 in.) dead bolt and inside thumb latch.

Electronic/Magnetic locking systems: Where installed, electronic locking systems will include an automatic “request to exit” sensor and a “push to exit” manual lock release switch. Refer to the NFPA Life Safety Code for details.

Install an Electronic Physical Access Control Systems (PACS) point for monitoring and controlling access to areas identified as requiring high or medium levels of assurance). This room needs to have a PACS system installed that feeds into the current C-Cure system.

Move Electrical panel out of space. Install a junction box in the ceiling and intercept electrical feeder and feeds. Re-install panel to interior adjacent wall. It is also acceptable to make the panel a junction box if a custom cover can be made with a finish look and identifying nomenclature.

Provide a small fireproof cabinet into that room that is vented Motion Intrusion Detector. An intrusion detection alarm system which detects entry into the room, and which broadcasts a local alarm of sufficient volume to cause an illegal entrant to abandon a burglary attempt. a. Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.

b. If VA determines that the contractor has violated any of the information confidentiality, privacy, and security provisions of the contract, it shall be sufficient grounds for VA to withhold payment to the contractor or third party or terminate the contract for default or terminate for cause under Federal Acquisition Regulation (FAR) part 12.

c. A contractor/subcontractor shall request logical (technical) or physical access to VA information and VA information systems for their employees, subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order.

d. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures.

SECURITY INCIDENT INVESTIGATION:

a. The term “security incident” means an event that has, or could have, resulted in unauthorized access to, loss or damage to VA assets, or sensitive information, or an action that breaches VA security procedures. The contractor/subcontractor shall immediately notify the COR and simultaneously, the designated ISO and Privacy Officer for the contract of any known or suspected security/privacy incidents, or any unauthorized disclosure of sensitive information, including that contained in system(s) to which the contractor/subcontractor has access.

b. To the extent known by the contractor/subcontractor, the contractor/subcontractor’s notice to VA shall identify the information involved, the circumstances surrounding the incident (including to whom, how, when, and where the VA information or assets were placed at risk or compromised), and any other information that the contractor/subcontractor considers relevant.

The term 'data breach' means the loss, theft, or other unauthorized access, or any access other than that incidental to the scope of employment, to data containing sensitive personal information, in electronic or printed form, that results in the potential compromise of the confidentiality or integrity of the data. Contractor shall fully cooperate with the entity performing the risk analysis. Failure to cooperate may be deemed a material breach and grounds for contract termination.
