About this file

This document contains a past and present performance questionnaire for evaluation of contractors on a Department of Veterans Affairs construction project. The questionnaire requests information on a contractor's performance on a prior project, to be completed by the contractor's references. It includes a section for the contractor to provide details of the reference project and their role. References are asked to rate the contractor's performance on a scale of 1 to 6 across various criteria such as managing personnel, quality control, scheduling, and responsiveness. References are also given opportunities to provide narrative explanations for poor ratings and state whether they would award the contractor another contract. The form requests the reference's contact information and signature upon completion.

A related federal contract opportunity notice provides details for a Department of Veterans Affairs solicitation to resolve site security assessment deficiencies at a medical center. The project scope involves demolition and construction required to prepare the site. The contract is valued between $5-10 million with an estimated 232-day period of performance. The solicitation will use a trade-off source selection process and is a 100% service-disabled veteran-owned small business set-aside. The point of contact is provided for any questions.
