DESCRIPTION

1. This Amendment adds Attachment 6 – Statement of Work.

2. This Amendment adds Attachment 7 – Northern and Central Zones Service Area Map.

3. Questions and Answers received 8-15-2022.

1. While Attachment 3 gives the evaluation factors of the technical proposal, I notice there is no statement of work attached. Is there a statement of work for this solicitation? This is very important because it will be difficult to develop a technical proposal which would meet the VA’s criteria.

1. The Statement of Work (Attachment 6) was added to the Solicitation by Amendment 1.

1. Does the Government intend to add power chairs to the CLIN item? The Bay Pines Healthcare System in particular has the incumbent assembling and delivering power wheelchairs to the CW Bill Young VA Hospital. This does not fit into the CLIN structure as currently written.

A: No, a CLIN will not be added for this.

1. What are the minimum and maximum space requirements for the storage of DME?

A: Contractor shall provide a minimum of 5,000 or maximum of 5500 square feet of storage space per medical center in each zone. Storage areas must conform to all applicable State, Local and National Fire Protection Association (NFPA) codes.

1. What are the immunization requirements for each service technician/driver serving the contract? We are aware of the requirement to track each employees’ COVID-19 vaccination status.

A: Tuberculosis Testing: All contractor personnel shall provide documentation of a Two-step Purified Protein Derivative (PPD) test performed within the past year and within ninety days of beginning at the contract.

Hepatitis B Vaccine: The contractor shall provide training or a self-study training module to its personnel, provide Hepatitis B vaccination series at no cost its personnel who elect to receive it.

1. What licenses and certifications does the vendor need to have?

A: See below.

1.The Contractor must be accredited and must demonstrate that it has met or exceeded Joint Commission Standards and provide documented evidence that it meets Joint Commission requirements for delivery, set up, repair, pick-up, and storage of durable medical equipment. 2.Proof of Insurance.

1. What are the requirements for physical locations (e.g., how far away from each VAMC)?

The Northern and Central Zone Service Map (Attachment 7) for coverage area of responsibility was added to the Solicitation by Amendment .
