PROPOSAL PREPARATION

Section L—Instructions, Conditions, and Notices to Offerors

1. OVERVIEW

1.1 This request for proposal (RFP) seeks proposals from Offerors interested in being selected to renovate kitchen Building 100 located at the Bay Pines VAMC, Bay Pines, FL. Offerors responding to this RFP are required to submit their proposals in accordance with the instructions contained in this section.

The kitchen renovation on the Bay Pines VAMC grounds will be a complete renovation of the 40-year old main kitchen in Building 100. This will not only modernize the kitchen’s infrastructure, but it will also support the transition to a room service model of meal delivery. This transition will enable Bay Pines’ Nutrition and Food Service to offer the Veteran a wider variety of meals from which to choose during their stay.

1.2 The purpose of these instructions is to establish uniform evaluation procedures for the proposals and the development of the Best Value Decision by the Source Selection Authority using the Trade-Off Process (See Federal Acquisition Regulation 15.101-1). In as much as the proposal shall describe the capability of the Offeror to perform the resulting contract, the proposal shall be specific and complete in every detail as well as be prepared simply and economically, providing a straightforward and concise description of capabilities to satisfactorily perform the contract.

The Government reserves the right to evaluate offers and award a contract without discussions with offerors, however, the offeror’s initial proposal should contain the offeror’s best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. If the Contracting Officer determines that the number of proposals that would otherwise be in the competitive range exceeds the number at which an efficient competition can be conducted, the Contracting Officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals.

1.3 For the purpose of this solicitation, the Government will award a single firm-fixed price contract.

2. PROPOSAL PREPARATION INSTRUCTIONS

2.1 Proposals must be complete, self-sufficient, and respond directly to the requirements of the solicitation. Any significant inconsistency, if unexplained, raises a fundamental issue of the offeror’s understanding and/or ability to perform the contract. This may be a basis for rejection of the proposal.

3. SUBMISSION INSTRUCTIONS

3.1 One electronic copy of all Volumes of the proposal (General Information, Technical, Past Performance and Price) shall be submitted to the Contracting Officer via email to Ashley.commiso@va.gov on or before the due date.

3.2 Faxed proposals are not permitted under any circumstances.

3.5.2 Factor 1 - Technical

3.5.2.1 Subfactor 1 - Technical Approach/Key Personnel

The offeror shall summarize its technical/management approach and team’s capability to obtain and retain suitably qualified personnel and maintain project continuity. At a minimum, the offeror shall address the following elements and, when viewed collectively, demonstrate its team’s ability to provide uninterrupted, high-quality work throughout the anticipated period of performance.

a. The offeror shall include a comprehensive narrative describing the construction activities as it relates to the construction work elements. The narrative shall clearly convey the Offeror’s understanding of the requirement and shall describe the work consistent with the schedule proposed. The narrative shall address any construction techniques such as sequencing, dependencies, and phasing.

b. Provide a list of Key Personnel for the proposed Team to be used to execute construction. At a minimum, this should include the project manager, superintendent, quality control manager, safety manager, and provide documentation (3-page limit for resumes) that they have the qualifications and experience to perform under the contract. The Offeror may submit other types of projects but must specifically explain the relevancy of those projects to the project that is being solicited in this RFP.

b. If the Offeror’s submission exceeds the three projects, any excess projects(s) will not be evaluated. The information that is required to be supplied is identified on the form provided below entitled “CONTRACTOR’S RELEVANT EXPERIENCE” (Attachment A). Offerors are not required to use the form itself as part of their proposal, but the information requested on the form must be in the Offeror’s proposal and should be in the same format as the form.

3.5.2.3 Subfactor 3 - Schedule

a. The proposal shall provide a proposed progress schedule. The progress schedule will be in a time scaled bar graph format. The horizontal axis will be scaled for time beginning with the Notice to Proceed and concluding with contract completion. The vertical axis will show the milestones and major portions of the contract work. Milestones should include such items as material submissions, submittal acceptance, partial and final inspections, punch list completion, testing, etc. All schedule items will show a start date and a completion date, and the critical path shall be clearly identified on the schedule.

b. The detailed schedule will indicate specific tasks with dates for each step of the process including: Mobilization; Demolition; Phasing; Provisions for overtime or shift work; Timing of relocation of existing utilities; Commissioning; Tests and final inspection. Task durations shall be expressed in calendar days.

3.5.3 Factor 2 - Past Performance

In addition to formal evaluations or if a formal evaluation is unavailable, the Offeror shall obtain the information using the PAST PERFORMANCE QUESTIONNAIRE (PPQ) form (Attachment B) included below. The Past Performance Questionnaire included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor 2, Past Performance. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment B), which will provide contract and client information for the respective project(s).

Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Ashley Commiso via email at Ashley.Commiso@va.gov prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. Also include performance recognition documents received within the last seven years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.

In addition to the above, the Government may review any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS), including Contractor Performance Assessment Reporting System (CPARS), using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), and any other known sources not provided by the offeror.

While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.

3.5.4 Factor 4 - Price (Price proposal shall be a separate file (clearly separated from the technical and past performance volumes.)

1. Completed Price Schedule on Standard Form (SF) 1442 (Total of Proposed Price of CLIN 0001)

Price proposals shall be submitted on SF 1442, completed, and signed by authorized individual(s) of the Offeror. Offers submitted in the name of a Joint Venture must be signed in accordance with the terms and conditions specified in the joint venture agreement as evidenced in the proposal. The price proposal shall encompass all costs associated with the requirements of the solicitation. The total proposed price for CLIN 0001 shall be rounded to the nearest dollar. Price will not be rated or scored but will be evaluated for fairness and reasonableness through the use of price analysis. In accordance with FAR 15.404-1(b)(2) the government may use various price analysis techniques and procedures to ensure a fair and reasonable price.
