STATEMENT OF WORK

Fuel Oil Tank Cleaning and Permanent Out-of-Service Safeguarding

Atlanta VA Health Care System

1. Scope

The contractor shall provide all labor, supervision, equipment, materials, transportation, and disposal services necessary to service an out-of-service above-ground storage tank (AST) at the Atlanta VA Medical Center by cleaning and decontaminating the tank interior; render the tank vapor-free at the time of testing and place it in a safe condition for permanent out-of-service status; safeguard the tank against tampering or unauthorized reuse; and provide documentation of final tank condition and waste disposition, if any. Physical removal of the tank is not included.

1.1 Place of Performance

Joseph Maxwell Cleland VA Medical Center, 1670 Clairmont Road, Decatur, GA 30033. The tank and work area are exterior to the main hospital building.

1.2 Period of Performance

The period of performance shall begin upon contract award or written authorization to proceed by the Contracting Officer and shall not exceed 30 calendar days. On-site work is anticipated to be completed in one to two workdays.

1.3 Contract Type

Firm-Fixed-Price.

1.4 Points of Contact

The Contracting Officer’s Representative (COR) and other technical points of contact will be designated in writing at or after award. The Contractor shall not accept technical direction from any individual other than the Contracting Officer or a designated COR acting within the limits of their delegation.

1.5 Tank Information

The tank is a single 4,500-gallon nominal capacity, horizontal, aboveground carbon steel storage tank (AST), approximately 8-10 feet in diameter and 20-25 feet long (dimensions to be field verified). Pumpable fuel has been removed from the tank; however, residual fuel, sludge, sediment, water bottom, oily film, vapors, or other secondary waste may remain. See below pictures.

2. Applicable Standards

The contractor shall perform the work in accordance with the applicable requirements of, including but not limited to:

• NFPA 30, Flammable and Combustible Liquids Code (provisions for above-ground tanks taken out of service);

• NFPA 326, Standard for the Safeguarding of Tanks and Containers for Entry, Cleaning, or Repair;

• OSHA 29 CFR 1910.146 (Permit-Required Confined Spaces) and 29 CFR 1910.120

(HAZWOPER), as applicable;

• EPA RCRA and used oil management standards (40 CFR Part 279), as applicable;

• DOT Hazardous Materials Regulations (49 CFR Parts 100–185), as applicable;

• Georgia EPD requirements; and the Authority Having Jurisdiction (AHJ).

Where requirements appear to conflict, the contractor shall notify the COR and Contracting

Officer before proceeding and shall comply with applicable law, permit conditions, and the direction of the AHJ. The contractor shall obtain or complete any permits, notifications, facility approvals, waste profiles, transportation documents, or disposal/recycling approvals required for the contractor’s work, including tank cleaning, waste handling, transportation, recycling, treatment, disposal, and permanent out-of-service safeguarding.

3. Specific Tasks

3.1 Pre-Work Coordination

• A pre-work site visit is not required but may be conducted if requested by the contractor or the Contracting Officer. If the contractor elects not to conduct a site visit, the contractor remains responsible for verifying means, methods, access, equipment needs, and site conditions necessary to complete the work within the awarded firm-fixed price.

• Submit a brief work plan, schedule, and site-specific health and safety plan (including a confined-space entry plan if tank entry is required) to the COR at least five (5) business days before mobilization.

• Provide not less than 72 hours advance notice of mobilization.

3.2 Waste Characterization (if applicable).

• Characterize and profile the #2 fuel oil residue if any, and each secondary waste stream

(oily water, sludge, sediment, rinse water, absorbents, used PPE) based on analytical data, generator knowledge, and applicable regulations.

• Submit waste profiles and receiving facility approvals before shipment of each waste stream, unless the receiving facility allows management under an existing approved profile or other lawful acceptance process. Copies shall be provided to the COR.

• The Government makes no representation as to whether any waste stream is hazardous or non-hazardous; the Contractor shall manage all waste in accordance with their actual regulatory status.

3.3 Secondary Waste Management (if applicable)

• Containerize, label, manifest, and transport each waste stream to a permitted facility for disposal or recycling, in accordance with applicable EPA, DOT, state, and local regulations.

• Discharge no fuel, wastewater, rinse water, or other residue to VA storm drains, sanitary sewer, soil, surface water, or any other location on or off VA property.

3.4 Tank Cleaning and Decontamination

• Clean and decontaminate the tank interior to remove residual fuel, sludge, sediment, water bottom, oily film, and other residue remaining after prior fuel removal. Cleaning methods may include vacuum extraction, pressure washing, wiping, mechanical ventilation, or other methods appropriate for the tank condition and consistent with applicable safety requirements.

• Collect, containerize, and manage all wastewater, rinse water, sludge, sediment, absorbents, used PPE, and other secondary waste generated during cleaning.

• No fuel, wastewater, rinse water, sludge, sediment, or other residue shall be discharged to VA storm drains, sanitary sewer, soil, surface water, or any other location on VA property.

3.5 Atmospheric Testing and Vapor-Free Status

• Perform atmospheric testing using properly calibrated instruments to document the tank’s condition at the time of completion. Testing shall include, at minimum, combustible vapor/LEL and oxygen concentration, plus any additional constituents required by the Contractor’s health and safety plan, the receiving facility, applicable standard, or the AHJ.

• Provide written documentation, signed by a qualified person, of atmospheric test results and the tank’s safe condition at the time of testing. Any certification of vapor-free status applies only at the time of testing.

3.6 Permanent Out-of-Service Safeguarding

• Upon completion of cleaning and atmospheric testing, safeguard the tank for permanent out-of-service status in accordance with NFPA 30 and NFPA 326.

• Secure all tank openings, manways, fill ports, gauge openings, and appurtenances disturbed during the work by capping, plugging, blind-flanging, locking, or otherwise mechanically securing access points as appropriate to prevent tampering, unauthorized reuse, or inadvertent filling. The tank vent shall remain configured in a safe manner unless otherwise directed by the AHJ.

• Apply durable, weather-resistant labeling at conspicuous locations on the tank indicating that the tank is out of service, empty, tested at completion, and not to be filled. Labeling shall include the date of the work.

• The contractor shall not cut, weld, hot-work, remove, demolish, or permanently alter the tank shell unless separately authorized in writing by the Contracting Officer.

3.7 Deliverables

• Pre-work plan, schedule, and site-specific health and safety plan (including confined-space entry plan, if applicable) prior to mobilization.

• Approved waste profiles or evidence of acceptance under an existing profile prior to shipment of each waste stream.

• Shipping papers, manifests, bills of lading, disposal tickets, recycling certificates, or other receiving facility documentation, as applicable, at the time of each shipment.

• Certificates of disposal, recycling, treatment or destruction, if applicable, within 30 calendar days of the final shipment.

• Atmospheric test records and signed documentation of the tank’s safe condition at the time of completion.

• A final completion report within 15 business days of completing on-site work, including:

quantities of secondary waste disposed, if any; receiving facility names, addresses, and

EPA ID or permit numbers, if applicable; final disposition path; dates of service; cleaning method; atmospheric test results; safeguarding measures performed; and final tank condition.

• Invoice documentation shall include, at minimum, the contract/order number, date of service, quantities of secondary waste removed (if any), waste streams generated if any, manifests or bills of lading, receiving facility documentation, and certification/disposition documents supporting the completed work.

4. Acceptance

All work is subject to review and acceptance by the COR. The Government will accept the work when the tank interior has been cleaned and decontaminated; atmospheric testing has been completed and documented; the tank has been documented as vapor-free or otherwise in a safe condition at the time of completion; permanent out-of-service safeguarding measures identified in Section 3.6 have been completed; all secondary waste generated by the work has been removed from VA property; and all required closeout documentation has been submitted and accepted.

5. Hours of Work

Monday–Friday, 7:30 AM – 4:30 PM. Weekend or after-hours work is permitted with COR approval.

6. Safety

The contractor shall comply with VA safety requirements; OSHA confined-space and

HAZWOPER standards as applicable; NFPA 30 and NFPA 326; lockout/tagout for contractor equipment; and DOT placarding and labeling for transportation. Spill kits and containment shall be staged at the work area for the duration of the work. The contractor is responsible for the immediate containment, cleanup, and lawful disposal of any spill caused by contractor operations, and for any required regulatory notifications. The contractor shall immediately notify the COR and designated VA site contact of any spill, release, leak, fire, injury, property damage, unsafe condition, or unexpected tank condition encountered during the work.

7. Site Conduct
