B.3 STATEMENT OF WORK
1. TITLE OF PROJECT:
Service and Preventative Maintenance contract for Bayer HealthCare LLC, Medrad Products at the Fayetteville VAMC and Fayetteville HCC.
2. BACKGROUND:
The Fayetteville VA Medical center has a requirement for a base plus 2 option year(s) for the renewal of service, preventative maintenance, and technical support of Over-Head Counterpoise systems which is a mount that connect the Stellant Flex to the ceiling, Stellant Flex contrast injection for CT and Solaris EP contrast for MRI.
3. STATEMENT OF WORK:
Vendor shall provide the following items available 8 AM - 5 PM M-F:
• Bayer EPM™
• Calibration per OEM specifications and procedures
• Software updates
• EPM™ certified part replacements
• Complete inspection and safety testing
• Onsite service by a Bayer certified field engineer
• Hardware system coverage Also included:
• Enhanced access to the myRadiologySolutions customer portal
• 24/7 Technical Support
• VirtualCARE® Remote Support

2. VA INFORMATION CUSTODIAL LANGUAGE. This entire section applies to all acquisitions requiring any Information Security and Privacy language.
b. Information made available to the contractor by VA for the performance or administration of this contract will be used only for the purposes specified in the service agreement, SOW, PWS, PD, and/or contract. The contractor shall not use VA information in any other manner without prior written approval from a VA Contracting Officer (CO). The primary clause used to define Government and Contractor data rights is FAR 52.227-14 Rights in Data – General.
k. Notwithstanding the provision above, the contractor shall not release VA records protected by Title 38 U.S.C. § 5705, Confidentiality of medical quality- assurance records and/or Title 38 U.S.C. § 7332, Confidentiality of certain medical records pertaining to drug addiction, sickle cell anemia, alcoholism or alcohol abuse or infection with Human Immunodeficiency Virus (HIV). If the contractor is in receipt of a court order or other requests for the above- mentioned information, the contractor shall immediately refer such court order or other requests to the VA CO for response.
3. ACCESS TO VA INFORMATION AND VA INFORMATION SYSTEMS. This section applies when any person requires access to information made available to the contractor by VA for the performance or administration of this contract or information developed by the contractor in performance or administration of the contract.
b. Contractors and subcontractors shall sign the VA Information Security Rule of Behavior (ROB) before access is provided to VA information and information systems (see Section 4, Training, below). The ROB contains the minimum user compliance requirements and does not supersede any policies of VA facilities or other agency components which provide higher levels of protection to VA’s information or information systems. Users who require privileged access shall complete the VA elevated privilege access request processes before privileged access is granted.
5. SECURITY INCIDENT INVESTIGATION. This entire section applies to all acquisitions requiring any Information Security and Privacy language.
b. To the extent known by the contractor/subcontractor, the contractor/ subcontractor's notice to VA shall identify the information involved and the circumstances surrounding the incident, including the following:
(1) The date and time (or approximation of) the Security Incident occurred.
(2) The names of individuals involved (when applicable).
(3) The physical and logical (if applicable) location of the incident.
(4) Why the Security Incident took place (i.e., catalyst for the failure).
(5) The amount of data belonging to VA believed to have been compromised.
(6) The remediation measures the contractor is taking to ensure no future incidents of a similar nature.
c. After the contractor has provided the initial detailed incident summary to VA, they will continue to provide written updates on any new and relevant circumstances or facts they discover. The contractor, subcontractor, and their employes shall fully cooperate with VA or third-party entity performing an independent risk analysis on behalf of VA. Failure to cooperate may be deemed a material breach and grounds for contract termination.

C.2 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

C.3 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed three (3) years.

A copy of the ACO Letter of Delegation will be furnished to the Contractor.

SECTION E - SOLICITATION PROVISIONS
ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES Provisions that are incorporated by reference (by Citation Number, Title, and Date), have the same force and effect as if they were given in full text. The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
E.1 INSTRUCTIONS TO OFFERORS – COMMERICAL PRODUCTS AND COMMERICAL SERVICES
Note that FAR PART 12 Procedures will be used.
1. TECHNICAL QUESTIONS
Offerors should submit all technical questions regarding this solicitation to the Contracting Specialist in writing, via email Jonathan.Lamb@va.gov on or before 10:00am, EST on December 3rd, 2025.

Telephonic (verbal) questions Will Not be addressed.
All responses to questions, which may affect offers, will be incorporated into a written amendment to the Request for Quote.
2. OFFER SUBMISSION
Offerors shall submit their quote in writing, via email Jonathan.Lamb@va.gov on or before 10:00am EST on December 5th, 2025. Quotes shall be assembled by the following Sections: each section shall be easily identified.
Section A - Offer Form (Standard Form 1449) Signed and dated, include the North American Industry Classification System (NAICS) code and small business size standard in Block 10. Unique Entity ID (UEI) Number shall be entered in the block with the Offerors name and address.
Section B – Pricing Pricing shall be submitted on Price-Cost Schedule.
Section C– Offerors Representations and Certifications All Offerors shall be registered and current in the System for Award Management (SAM) database at time of offer submission and maintain active registration until contract award, completion of performance, and final payment.
System updates may lag policy updates. The System for Award Management (SAM) may continue to require entities to complete representations based on provisions that are not included in agency solicitations. Examples include 52.222-25, Affirmative Action Compliance, and paragraph (d) of 52.212-3, Offeror Representations and Certifications—Commercial Products and Commercial Services. Contracting officers will not consider these representations when making award decisions or enforce requirements. Entities are not required to, nor are they able to, update their entity registration to remove these representations in SAM.
Failure to submit a complete quote may result in a technically unacceptable determination.
3. Amendments:
Amendments to this solicitation will be posted on Contract Opportunities. Paper copies of the amendments will NOT be individually mailed. Offerors will be notified by Contract Opportunities of any new amendments that have been issued. No other notifications of amendments will be provided. Potential Offerors are advised that they are responsible for obtaining and acknowledging any amendments to the solicitation. Failure to acknowledge an amendment may result in your quote being considered unacceptable.
4. DUE DATES
All offers shall be submitted by 10:00am est on December 5th, 2025 in writing, via email Jonathan.Lamb@va.gov
5. AWARD BASIS
Note that FAR Subpart 12 Procedures will be used. The Government intends to award a contract resulting from this solicitation to the responsible offeror whose quote, conforming to the solicitation, quotes the lowest price, whose quoted price does not exceed the amount of funding available for the procurement, and whose quoted price is found to be reasonable. The evaluation factors used for this award are technical, past performance, and price.
Factor 1: Technical/Management Approach The evaluation process will consider whether the quote demonstrates a clear understanding of the technical features involved in meeting the requirements of the solicitation.
Technical:
Provide Original Equipment Manufacturer (OEM) Bayer Healthcare LLC service provider authorization letter for Medrad systems.

The Offeror shall provide three references: federal and private sector (if applicable). Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last three years. List the following information for each contract:
