B.3 STATEMENT OF WORK
Animal Clinic Epoxy Flooring Building 500 Hunter Holmes McGuire VA Medical Center Richmond Virginia 23249
1. General Requirement--The Contractor will provide all transportation, materials, parts, tools, specialized equipment, labor, supervision, and technical expertise grind, and prep floors and install Epoxy flooring at 1201 Broad Rock Blvd Richmond, VA 23249
2. Coordination. The engineering Carpentry shop will serve as the focal point and point-of-contact (POCs) for this contract. The Point Of Contact for this service TBD
2.1. The contractor must pre-coordinate and preschedule work with the POCs. Project will be scheduled in 4-6 phases approx. 2,700 Square feet in each phase. This work can only be performed during normal business hours, Monday thru Friday 7:00 A.M. to 5:00 P.M.
3. Government Provided Tools, Materials and Equipment. The Federal Government will not provide any tools, parts, materials and equipment. There will be power (110) power and 220-volt, 30-amp single phase supplied to the jobsite. This is a turn-key project with the vendor providing all necessary tools, parts, materials, and equipment.
4. Specifications. The following specifications will apply: 8-10 mils of low VOC primer/ binding agent. 10-12 mils of a high solid general purpose pigmented epoxy coating with silica additive for a nonskid finish and water borne aliphatic urethane finish topcoat.
Report to the Hunter Holmes McGuire VA Medical Center, Engineering Carpentry Shop POC before starting work each day.
5.1. All work will be accomplished in-accordance with applicable Life Safety and OSHA codes and regulations, Available in Additionally, the Department of Veteran Affairs guidelines and policies will be adhered to.
5.2. Provide moisture slab test
5.3 Diamond grid concrete, with an external dust control HEPA vacuum to remove all old existing coating, to clean concrete.
5.4. Minor spot repairs and patching of floor cracks.
5.5 All sloping to be in the concrete substrate, and to existing drains.
5.6. All work-related debris will be carried off station by the contractor.
6. Other Station Rules and Guidelines. The Contractor will adhere to the following station rules.
6.1. This is a hospital environment. The Contractor and his personnel are expected appropriately dressed (preferably uniformed) and to conduct themselves professionally.
6.2. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on campus grounds. Failure to follow these regulations can result in removal of the worker or contractor from the facility and/or a US Magistrate’s ticket and fine.
6.3. Parking will be in designated lots. Consult with the POC as to allowable parking and material drop-off.
6.4. The contractor will be responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the hazards found at the site. The contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation.
6.5. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect federal facilities. The contractor will provide to the POC a list of employees expected to work on this contract.
6.6. The U.S. Department of Veterans Affairs and the U.S. Department of Health and Human Services requires that all contractor personnel coming on this facility to complete the Veterans Health Administration’s Privacy Policy Training the day of arrival, in 2k-113 Safety’s office in this facility. The Contractor will submit the certificate, to be completed, at the end of the Microsoft Training PowerPoint Presentation for each person assigned to this contract who comes on this facility to work. A company roster certifying course completion may be substituted for individual certificates.
6.8. All contractors’ personnel, including subcontractors, shall be required to undergo a COVID-19 screening. Each person is required to complete the Coronavirus screening upon entering the facility and worksite thereof and/or while on the hospital campus.
B.4 SPECIAL CONTRACT REQUIREMENTS
A. WORK HOURS OR HOURS OF OPERATION
The services covered by this contract shall be furnished by the contractor as defined herein. The contractor shall not be required, except in case of emergency, to furnish such services on Federal Holidays or during off duty hours as described below.
Normal working hours are Monday through Friday, 8:00am – 4:30pm, excluding federal holidays which are as follows:

Any other day specifically declared by the President of the United States to be a federal holiday.
When one of the holidays falls on Sunday, the following Monday shall be observed as a Federal Holiday. When a holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday.
B. CONTRACTOR EMPLOYEES
A full background check is not required for this position; however, all contract employees must bring photo identification and advance notice from the requiring service to obtain a VA Badge in order to work on a VA Facility. Upon award, the CO shall provide the contractor with detailed instructions on fulfilling security requirements.
All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Governmental installation shall abide by all security instructions and directives of the Department of Veteran Affairs. Employees are responsible for safeguarding all Government property provided for contractor use.
The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal Background Investigation (CBI).
Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this requirement. Furthermore, nothing in this requirement shall excuse the contractor from proceeding with the contract as required.
C. BILLING AND PAYMENT
The contractor shall submit payment requests in electronic form via VA’s Electronic Invoice Presentment and Payment System http://www.fsc.va.gov/fsc/einvoice.asp.
Invoices shall be submitted upon completion of contract requirement. The following information must be included on all submitted invoices. Invoices submitted without the following information may be rejected for payment. Invoices shall include at a minimum:

D. CONTRACT ADMINISTRATION
Notwithstanding the Contractor's responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor.
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that shall affect price, quantity or quality of performance of this contract.
The Point Of Contact (POC) shall be responsible for the overall technical administration of this contract as outlined in the SOW.
In the event the contractor effects any such change at the direction of any person other than the contracting officer without authority, no adjustment shall be made in the contract price to cover an increase in costs incurred as a result thereof.

E. VAMC SMOKE FREE POLICY
In striving to provide a healthy and safe environment at our facilities, VHA is implementing a smoke-free policy for all VHA health care facilities and property assigned to VHA that is under the charge and control of VA.
Under VHA Directive 1085, all VHA health care facilities (including CBOCs and HCCs) will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019.The policy applies to cigarettes, cigars, pipes, or any other combustion of tobacco, including but not limited to electronic or e-cigarettes, vape pens or e-cigars.
Although VA has historically permitted smoking in designated areas, there is growing evidence that smoking and exposure to secondhand and thirdhand smoke creates significant medical risks, and risks to safety and direct patient care that are inconsistent with medical requirements and limitations.

B.5 COVID -19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS
VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors
1. Contractor employees who work in or travel to VHA locations must comply with the following:
a. Documentation requirements:
1) If fully vaccinated, shall show proof of vaccination.
i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
2) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.
3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.
4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.

3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.

As used in this clause— "Postconsumer fiber" means— (1) Paper, paperboard, and fibrous materials from retail stores, office buildings, homes, and so forth, after they have passed through their end-usage as a consumer item, including: used corrugated boxes; old newspapers; old magazines; mixed waste paper; tabulating cards; and used cordage; or
(2) All paper, paperboard, and fibrous materials that enter and are collected from municipal solid waste; but not
(3) Fiber derived from printers' over-runs, converters' scrap, and over-issue publications.
(b) The Contractor is required to submit paper documents, such as offers, letters, or reports that are printed or copied double-sided on paper containing at least 30 percent postconsumer fiber, whenever practicable, when not using electronic commerce methods to submit information or data to the Government.

(a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside.
(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

(a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that—
(1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows:

(3) If VA determines that an SDVOSB/VOSB awarded a contract pursuant to 38 U.S.C. 8127 did not act in good faith, such SDVOSB/VOSB shall be subject to any or all of the following:

The Contracting Officer may, at their discretion, require the Contractor to demonstrate its compliance with the limitations on subcontracting at any time during performance and upon completion of a contract if the information regarding such compliance is not already available to the Contracting Officer. Evidence of compliance includes, but is not limited to, invoices, copies of subcontracts, or a list of the value of tasks performed.
(c) The offeror further agrees to cooperate fully and make available any documents or records as may be required to enable VA to determine compliance with the limitations on subcontracting requirement. The offeror understands that failure to provide documents as requested by VA may result in remedial action as the Government deems appropriate.
(d) Offeror completed certification/fill-in required. The formal certification must be completed, signed, and returned with the offeror’s bid, quotation, or proposal. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

SECTION E - SOLICITATION PROVISIONS
E.1 INSTRUCTION TO OFFERORS
FAR PART 13 SIMPLIFIED ACQUISITION PROCEDURES
1. QUOTE SUBMISSION
Quoters shall submit their quote via email to Warren A. Pennil at warren.pennil@va.gov by 12:00AM, EST, June 13, 2022. Quotes shall be assembled by the following Sections; each section shall be easily identified.
Section A - Quote Form (Standard Form 1449) Signed and dated, include the North American Industry Classification System (NAICS) code and small business size standard in Block 10. Data Universal Numbering System (DUNS) Number shall be entered in the block with the Quoters name and address.
Section B – Pricing Pricing shall be submitted as requested in the Schedule of Services. Total price will be evaluated by the Government. Proposed price should reflect a clear understanding of the requirements and is consistent with the various elements of the quote.
Section C– Quoters Representations and Certifications A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the quoter shall complete electronically). A quoter shall complete only paragraph (b) of this provision if the quoter has completed the annual representations and certifications electronically at https://beta.sam.gov/
All Quoters shall be registered and current in the System for Award Management (SAM) database at time of award.
All Quoters MUST be a verified Service-Disabled Veteran Owned Small Business (SDVOSB) vendor at the time of quote submittal. To be considered a SDVOSB, the company must be listed as verified in the vendor information pages in the VIP database, https://vip.vetbiz.gov .
Section D – Technical/Management Approach Quoters shall submit a quote that addresses all aspects of the Statement Of Work (SOW).
The quoter shall identify any subcontractors proposed under this solicitation.
