About this file

This is a Past Performance Questionnaire template issued by the Department of Veterans Affairs to evaluate contractor performance on a contract for "Conversion of Analog Signals to Digital Signals" in accordance with a Statement of Work.

The questionnaire is designed to be completed by references provided by an offeror and submitted to VA evaluators. Respondents are asked to assess the offeror's past performance across five key areas: Delivery and Scope (work completion), Quality (adherence to standards), Timeliness (deadline compliance), Financial Performance (pricing and payment consistency), and Compliance and Management (obligation fulfillment and issue resolution). Each category uses a four-point rating scale: Exceptional (performance exceeded requirements with no issues), Satisfactory (met all requirements with minor issues and effective corrective actions), Unsatisfactory (did not meet requirements with significant problems and ineffective corrective actions), or Not Applicable (unable to provide meaningful assessment). Respondents providing "Exceptional" or "Unsatisfactory" ratings must provide brief narratives explaining their rationale. The questionnaire includes a comments section and requires the respondent to provide their company name, name, title, email, and completion date. All information provided is classified as Evaluation Information, prohibited from disclosure, and exempt from Freedom of Information Act release.
