VOLUME I – TECHNICAL PROPOSAL QUESTIONS:
1. Section 1 – Technical Capability. Are we to insert completed “Exhibit A – Technical Proposal Data” as a separate attachment or are we to insert the verbiage from Exhibit A into the proposal itself?
5. Volume I - Technical Proposal (Three-Ring Binder)
Section 1 Technical Capability Answers to Exhibit A
Section 2 Past Performance Answers to Exhibit B, and Answers to Exhibit C
Also, the categories listed on Exhibit A (Capability and Experience, Organization and Specific Personnel, etc.) don’t correspond sequentially with Subfactors 1, 2 and 3 under Evaluation Guidance found in Procedures for Submittal of Offers. Which are we to use as guide/format for drafting our proposal?
The “categories” in Exhibit A do not have to match sequentially with Subfactors 1, 2, and
3. The information in Exhibit A is used to evaluate the Subfactors.
2. With respect to Exhibit A, is the requested Organizational Chart listing company personnel and roles as it relates to the Seed Project only? It will be difficult otherwise to identify key personnel for other projects that may potentially be awarded under the MATOC without a defined SOW.
Both permanent company personnel (president, vice, etc.) and SEED project personnel.
Attachments to your Exhibit A:
1. Organization Chart with brief relevant details of individual experiences/qualifications
2. Tentative Schedeule
3. Quality Control Synopsis
4. Safety Plan Synopsis
3. Section 2 – Past Performance. Are we to insert completed Exhibits B and Exhibits C as separate attachments or are we to insert the verbiage from those Exhibits into the proposal itself?
Volume I - Technical Proposal (Three-Ring Binder)
Section 2 Past Performance Answers to Exhibit B, and Answers to Exhibit C
Also, instructions state that this section shall not exceed 12 total pages. This is not realistic if we are to include two Exhibit Bs for each major subcontractor as well as the Prime Contractor. PLEASE CONFIRM
Technical and Past Performance are two separate Evaluation Factors. Yes 40- page limit for Technical and 12-page limit for Past Performance to be submitted in accordance with
Exhibit B.

The document should be written to show the team’s construction performance, qualifications with respect to health care construction projects, i.e. new hospital construction, hospital renovations, demo projects, and projects generally associated with the maintenance and expansions hospital campuses. The past performance section shall not exceed twelve (12) total pages.
Are Subcontractors’ completed Exhibit Bs included as part of their completed/executed Exhibit C – Subcontractor Consent Form (for purposes of page count)? PLEASE
CONFIRM
Volume I - Technical Proposal (Three-Ring Binder)
Technical proposal will not exceed 40 pages. Pages beyond the limit specified will NOT be read or evaluated. Front and back side use of a single page will count as two (2) pages.
Company advertising literature shall not be included. The 40-page limit is inclusive of
ALL pages of the proposal IN ITS ENTIRETY. Exhibit C – Subcontractor Information and Consent Form and Exhibit F - Past Performance Questionnaires are the ONLY exceptions to the 40-page limit.
4. Are the major subcontractors that we are to identify with respect to the Seed Project only? Obviously not knowing which projects will fall under the MATOC, we cannot foresee at this point which major subcontractors may be required. PLEASE CONFIRM
Major sub-contractors for the SEED project as well as major sub-contractors that the prime works with on a regular basis and who will be used during the MATOC. The proposed subcontractors, applicable for multi-discipline consideration, must be verifiable, and information provided indicates a good professional relationship with Sitework, General Construction, Mechanical Construction, and Electrical Construction.
5. Page limitation. Is the 40-page limit inclusive of both Section 1 and Section 2?
