Elevator Statement of Work
VISN 4
1. PURPOSE
The Veteran Integrated Service Network 4 (VISN 4) has a requirement for all labor, equipment, supplies, management and travel to perform Elevator Maintenance Inspection and Elevator Safety Testing Services at nine VA Medical Centers in Pennsylvania and Delaware.
The Contractor shall plan, schedule and coordinate inspection services with designated VISN 4 personnel to provide inspection and safety testing services for 166 elevators, dumbwaiters, and other vertical transport equipment in accordance with this Statement of Work (SOW). A list of equipment requiring inspection and safety testing services is provided in Attachment 1- ELEVATOR EQUIPMENT. Elevator equipment may be added, removed or replaced from the contract at VISN 4 discretion at any time during the contract performance period. Any changes to the contract as a result of equipment changes shall be reflected in a mutually agreed upon written contract modification.
2. GENERAL REQUIREMENTS
a. The Contractor shall not perform any maintenance services on elevator equipment covered by this contract at any time during the entire contract period of performance.
b. The Contractor shall meet with the Contracting Officer’s Representative (COR), or designated VISN 4 representative, at the start of each inspection to review the status of all elevator equipment. The COR shall provide all required elevator maintenance documentation to the Contractor for use during the inspection.
c. The Contractor shall meet with the COR or designated VISN 4 representative at the conclusion of the inspection to present their findings. The inspector shall detail any problem areas encountered and recommend corrective action
3. Upon completion of each inspection, the Contractor shall obtain the signature of the COR or designated VISN 4 representative on the Elevator Inspection Report form. The report may be handwritten at the time of the inspection but must be submitted in typed format within two weeks after the completion date of the inspection. Contractor shall provide two full paper reports for each facility and one digital copy for each facility for each inspection
a. In the event that any elevator equipment fails an inspection the Contractor shall immediately report the failure to the COR and shall submit a written report within 48 hours which lists the reason(s) for failure, the defects found and the recommended corrective action.
b. The Contractor shall immediately notify VISN 4 of any deficiencies discovered while performing inspection services that may jeopardize elevator passenger safety or which risks damage to elevator equipment. A written report which lists the defects found and the recommended corrective action shall be submitted to the COR within 24 hours.
4. STANDARDS
All safety inspections shall be made in accordance with the latest edition of the American Society of Mechanical Engineers (ASME) Safety Code for Elevators and Escalators (ANSI/ASME A17.1) hereafter referred to as the A17.1 Code. The Contractor shall use the A17.1 Code as the minimum requirement when performing inspection services covered by this contract. The ASME Guide for Inspection of Elevators, Escalators and Moving Walks (ANSI/ASME 17.2) shall be used as the guide for inspection and test procedures, hereafter referred to as the A17.2 Code.
5. SAFETY TESTS AND MAINTENANCE INSPECTIONS
a. Safety Tests - The contractor shall conduct no-load, full-load and special elevator equipment safety tests as required by, and in accordance with, the A17.1 and A17.2 Codes. The tests shall include, but not be limited to, semi-annual, annual and 5-year tests as required by the A17.1 Code. The Certification of Elevator Inspection form shall be used to document safety inspections.
b. Maintenance Inspections - The Contractor shall utilize the A17.1 and A17.2 Codes to conduct semi-annual maintenance inspections on each item of elevator equipment for evidence of proper maintenance, adjustment and repair actions conducted by VISN 4 elevator maintenance Contractor. Inspections will also include a review of the elevator maintenance Contractor’s preventative maintenance check charts to verify that they are up to date and properly annotated. Inspection results will be documented in the Elevator Inspection Report form (Attachment 2). Any deficiencies shall be noted in the remarks block of the inspection report form.
6. SCHEDULED SERVICES
The Contractor shall arrange site visit dates and times with the COR a minimum of three weeks prior to a scheduled test/inspection service due date. Unless otherwise directed by the COR, all services shall be performed during normal business hours; Monday – Friday, 7:00 AM – 3:30 PM, excluding federal holidays. If the COR requires a site visit outside of normal business hours to minimize interruptions of service, such work shall be done at scheduled pricing at no additional cost to the Government.
Any Inspections that require more than one elevator being shutdown in an elevator bank, will have to be performed on off peak hours (7:00am – 4:30pm)
7. CONTRACTOR QUALIFICATIONS
The Contractor shall assign qualified elevator equipment inspectors to perform the requirements listed in this SOW and shall not change assigned inspectors without prior approval by VISN 4. Contractor personnel that are designated to perform services shall possess current American Society of Mechanical Engineers – Qualified Elevator Inspector (ASME-QEI) certification and shall provide proof of certification to the COR prior to performing services.
8. CONTRACTOR EMPLOYEE CONDUCT
The contractor shall be responsible for ensuring that its employees meet satisfactory standards of conduct, appearance and integrity, and shall take corrective actions with respect to its employees as necessary. The Contractor’s employees shall abide by VISN 4 policies including, but not limited to, parking, possession of weapons restrictions, smoking and policies allowing the search of enclosed containers and vehicles by VISN 4 Police Service.
The Contractor shall take all precautions necessary to protect persons and property from injury or damage. The Contractor shall be responsible for injury to its employees or others, and for damage to personal or public property that occurs due to negligence of its employees.
9. VA HEALTHCARE DIRECTIVES
The Contractor shall comply with the requirements of the following VA Healthcare Directives:
0. Tuberculosis Screening- VHA Directive 2011-036, Safety and Health During Construction. Contractor must certify annually to the COR(s) that all Contractor employees working at VISN 4 facilities have a documented screening for latent Tuberculosis within the last 12 months. If screening for latent Tuberculosis is positive, clearance from a physician is required prior to working at VISN 4. Contractor employees that fail to meet this requirement will not be permitted to work at VISN 4 facilities and may be asked to leave VISN 4 property without notice until the requirement is met.
0. Seasonal Influenza Vaccination - VHA Directive 1192, Seasonal Influenza Prevention Program for VHA Health Care Personnel. Contractor will certify annually to the COR(s) that all employees working at VISN 4 facilities have received a seasonal influenza vaccination.
10. VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors
a. Contractor employees who work in or travel to VHA locations must comply with the following:
1) Documentation requirements:
a) If fully vaccinated, shall show proof of vaccination.
i.) NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination.
b) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor’s order or an FDA approved over-the-counter test.
c) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel.
d) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
b. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates.
1). Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
c. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
