About this file
This is a Past Performance Questionnaire (PPQ) template form designed for Architect-Engineering (AE) firms to submit references documenting their past performance on relevant projects. The form requires contractors to provide firm information (name, CAGE code, address, DUNS number, contact details) and specify their role as prime contractor, subcontractor, joint venture, or other entity, along with the percentage of project work performed. Contract details to be documented include contract number, type (Firm Fixed Price, Cost Reimbursement, or other), title, location, award date, completion date, original and final contract prices, and a description of project complexity (High, Medium, or Routine) with explanation of relevance to the current solicitation.
The evaluation section requires client references to rate contractor performance across eight major categories using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Available): Quality (technical data preparation, quality standards compliance, problem resolution, quality control); Schedule/Timeliness (delivery schedule compliance, resource utilization); Customer Satisfaction (end-user satisfaction, staff cooperation, contractor professionalism, overall satisfaction); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, requirement changes); Cost/Financial Management (meeting contractual prices, innovative cost-reduction methods, invoice timeliness, accounting system adequacy, termination history, financial stability); Safety/Security (safety compliance and security requirements); and General factors (emergency response capability, contractual compliance, rehire willingness, overall performance rating). Clients must provide narrative explanations for any unsatisfactory or marginal ratings and complete sections addressing strengths, weaknesses, and deficiencies in a remarks section.
