STATEMENT OF WORK (SOW) Project # 561A4-26-500 Repaving of CLC Loading Dock Lyons Campus

1. Contract description.

This contract covers the demolition and replacement of approximately 1,600 sq ft of 12- inch-thick concrete, including the removal of old steel bollard bases and proper disposal of all debris into a contractor provided dumpster. Due to the requirement that the CLC loading dock remain operational during construction, the concrete portion of the scope shall be completed in two equal phases, with temporary fencing installed around the active work area to separate it from operational space. Phase 2 shall not commence until Phase 1 is fully cured, sealed, and protected, and the area is returned to a functional condition. The scope includes scanning for underground utilities, subgrade compaction, installation of reinforced concrete with control joints and a broom-finished, sealed surface, ensuring durability for heavy loads and proper drainage. Additionally, the contract includes demolishing an 11’ by 25’ section of asphalt and 22 linear feet of curb. Scope requires scanning for underground utilities and coordinating generator shutdown for safe demolition practices. Materials removed should be tested for contamination and in the case that contamination is found, remediation will be performed per VA regulations before new construction proceeds. The work concludes with the installation of new asphalt and curb, proper grading and compaction, installation of 3 steel bollards behind new curb, and a final inspection to ensure compliance with all project specifications and local standards.

All demolition debris will be removed and disposed of at approved contractor provided dumpster, leaving the site clean and ready for use. Contract should include all work necessary during normal business hours, after-hours (second or third shift), weekends, and federal/state holidays as required to meet project deadlines and minimize disruption to facility operations.

2. Background.

Behind the Community Living Center at the Lyons campus is a 1,600 sq ft flab of concrete that has a depth of approximately 8 inches. The concrete must be removed and replaced due to damage and integrity concerns. The contractor must submit a 10’x10’ layout to be approved by COR including control joints every 10 feet to prevent future cracking. To limit construction impact on operations of the CLC, the concrete portion of the scope will be 2 phases split in half.

Furthermore, adjacent to the generator there are damaged pieces of curb as well as asphalt that is pooling water attributing to a growing number of insects that can potentially migrate into the building and cause health concerns. The curb and asphalt must be demolished safely and reinstalled at a slope that forces the water to drain properly into the water basin. Two (2) catch basins must be removed and reinstalled at the same size, work to include any asphalt rebuilding or patching that is necessary.

Behind the curb, rebuild to include the procurement and installation of 3 steel bollards to protect the generator.

The work will encompass the entire construction process - ranging from demolition, scanning, testing, installation, as well as certifying.

3. Scope.

The contractor shall provide all necessary labor, including premium evening, weekend and holiday hours, materials to demolish and remove approximately 1,600 square feet of existing 12-inch-thick concrete, including old steel bollard bases, with all debris disposed of in contractor provided dumpster. The scope covers scanning of concrete for underground utilities, phasing of work, subgrade compaction and grading, installation of a reinforced concrete slab while using a wire mesh system and/or rebar system with control joints, a broom-finished, sealed surface to ensure durability and proper drainage for heavy-duty use. The contract also encompasses the scanning for utilities, coordination of generator shutdown, and demolition of an 11’ by 25’ section of asphalt pavement and 22 linear feet of curb. All removed materials will be tested for contamination and, if necessary, remediation will be performed in accordance with all appropriate VA regulations. The work includes installation of new asphalt pavement and curb, grading and compaction to match existing conditions, and a final inspection to verify compliance with project specifications, state standards, as well as VA regulations. Two (2) Catch basins must be replaced and scope will include patch, repair, or rebuilding of asphalt necessary for catch basin installation.

All demolition debris will be removed and properly disposed of, leaving the site clean and ready for use. Contractor must comply with all applicable VA specifications for the VA NJ Healthcare System, Lyons campus.

Contractor will also have to provide quarterly waste manifest, to the project COR for record keeping.

4. Specific Tasks.

A. The scope of work shall include but is not limited to the following main items:

Concrete Repaving

1. Properly phase construction into 2 separate identical scopes of work to minimize CLC disruption of operations

2. Scan concrete for underground utilities to ensure safe demolition (2 separate scans, 1 per phase). Scan report must be submitted to the VA for review and a VA representative should be present at the time of scanning.

3. Demolish and remove approximately 1,600 sq ft of concrete to a depth of 12 inches using jackhammers. Dispose of all debris into a contractor provided dumpster including timely swaps - and leave area clean. Include demo of old steel bollard bases. (2 separate demolition phases)

4. Compact and grade subgrade after concrete removal to achieve uniform surface and proper drainage. Compact soil to at least 95% of maximum density. (2 separate times)

5. Install control joints to minimize random cracking in the concrete slab. Joints shall be placed in a grid pattern, with panels as square as possible and not exceeding 10 feet by 10 feet. Joint depth shall be at least 1/4 of the slab thickness (minimum 3 inches for a 12-inch slab) to accommodate heavy loads such as trucks and compactors (once per phase)

6. Install formwork to create an approved pattern of 10 ft by 10 ft sections (separated by control joints) throughout the slab area. After concrete placement and curing, remove forms and fill all joints between sections with flexible joint sealant (caulk) to ensure proper movement and prevent water infiltration. (once per phase)

7. Install both welded wire mesh and ½-inch rebar in a grid pattern spaced 12 inches on center each way throughout the slab. Ensure reinforcement is properly supported and positioned per design requirements. (once per phase)

8. Provide 4,500 PSI concrete mix with fiber mesh reinforcement for added crack resistance and durability. (once per phase)

9. Vibrate concrete using a poker or immersion vibrator to eliminate trapped air, compacting the mix to distribute aggregates evenly. (once per phase)

10. Apply a broom finish to the concrete surface after initial troweling and when the slab is firm enough to hold an imprint. Use a stiff-bristle broom and drag it evenly across the surface in one direction with consistent pressure to create a textured, slip-resistant finish. (once per phase)

11. Apply 2 layers of sealer, begin with a silicate densifier and finish with a silane/siloxane sealer. (once per phase)

Asphalt/ Curb Renovation

1. Scan beneath the asphalt adjacent to the generator to determine the depth and location of all piping and utilities to ensure safe demolition. Mark out exact locations and develop a plan to chip asphalt in sensitive locations rather than sawcut.

2. Coordinate with site personnel to schedule and execute a full shutdown of the generator before any demolition work begins to prevent hazards

3. Demolish and remove 11’ by 25’ section of asphalt pavement, including all base and sub-base layers as required.

4. Following asphalt removal, demolish and remove 22 linear feet of curb at the control joint, using saw-cutting for clean separation and to minimize damage to adjacent surfaces.

5. Remove and safely store asphalt, subgrade, and remaining materials samples from the work area to test for contamination from prior oil leaks.

6. If contamination is detected, implement remediation measures in accordance with environmental regulations before proceeding with new construction.

7. Install new asphalt pavement in the demolished area, ensuring the finished surface is set 2 inches higher at the curb to provide a positive slope for drainage away from the structure and toward the catch basin.

8. Reinstall new curb to match the existing curb’s depth, alignment, and profile, ensuring a seamless transition with adjacent curb sections.

9. Compact and grade all disturbed areas to match surrounding grades and ensure proper drainage.

10. Remove all demolition debris from the site and dispose of it at an approved facility.

11. Conduct final inspection to verify that new asphalt and curb installation meets project specifications and local standards for slope, compaction, and finish.

12. Replace 2 catch basins matching size of existing ones.

5. Performance Monitoring (if applicable).

Work shall be coordinated with facility through the Contracting Office Representative (COR). COR will monitor the contractor’s work performance on daily basis, and the contractor is required to submit daily progress report to the COR.

6. Security Requirements.
