1. The Contractor shall provide all labor, supervision, supplies, materials, equipment and transportation to perform all operations in connection with the Integrated Rodent and Pest Management (IPM) program tasks as specified herein. All work is to be performed by qualified/certified applicators in accordance with all requirements and regulations, Federal, State and local, governing the service requested and in accordance with the contractor’s Procedures Manual and Quality Control Manual. Provider of service shall be available to be on-site within four (4) hours of requested service by the Contracting Officer Representative (COR).

2. All work associated within this Statement of Work shall be completed during normal business hours, unless otherwise noted by emergency or in Attachment Four. Normal business hours are typically 7:00 am to 4:30 pm Monday through Friday, exclusive of Federal Holidays (or other mutually agreed upon time) on the designated day as agreed upon by the COR and the Contractor.

Federal Holidays observed by the VAMC are:

New Year’s DayLabor Day
Martin Luther King DayColumbus Day
President’s DayVeterans Day
Memorial DayThanksgiving Day
Independence DayChristmas Day

3. VACO’s pest management policy the 1850.02 mandates that certain areas are inspected weekly, these areas include pharmacy, kitchens, canteens, eating areas which include breakrooms, loading docks and trash areas, laundry, ORs and SPS. However, areas such as waiting rooms, patient wards and other public areas are inspected bi-weekly or as needed.

Due to the size of each facility, it would be much more thorough and efficient to have each site inspected 3x per week, applying treatment when necessary.

Integrated pest management is basically broken down into steps.

· Step 1 Training staff on avoiding enticing pests into an area such as not eating at ones desk and leaving the crumbs.

· Step 2 – Inspection and identifying issues allowing pests into an area such as loose baseboards, missing ceiling tiles, missing door sweeps etc..

· Step3 – identifying and treating for the identified pests.

· Step 4 – Follow-up and reinspection to verify the effectiveness of the treatment.

4. Place of Performance: New York Harbor VA Health Care System requires Pest Control services for the following locations:

1. Manhattan Campus 423 E 23rd Street New York, NY 10010

2. Brooklyn Campus 800 Poly Place Brooklyn, NY 11209

3. St. Albans Community Living Center 179-00 Linden Blvd Jamaica, NY 11425

5. Period of Performance: The period of performance is estimated 05/01/2026 – 04/30/2027, with the provision of four option years.

6. Contract Type: Firm Fixed Price. Single Award.

B. AUTHORITY OF APPLICATOR(s) Certified applicators are required in the performance of the work under this contract. Each applicator designated by the Contractor to perform work under the contract will furnish a copy of his/her state certification (showing categories). This copy shall be furnished to the Contracting Officer (CO) and COR for approval and prior to reporting for work. All applicator’s uniforms shall visibly show the individual’s name and the company name so as to be easily recognizable at all times. In cases where the Contractor has two or more persons reporting to work under the contract, one of these shall be designated as a supervisor and must be fully certified in all pest management categories in which work is to be performed. Non-certified employees may assist the supervisor in the performance of the work as long as they are in immediate contact with and visible to the supervisor. The supervisor will assume all responsibility for their work. On each visit, the applicator(s) shall report to the Manager, Environmental Management Service, (EMS), prior to performing any work.

NOTE: Certification shall mean a current certificate of competency in pest management in any and all categories or sub-categories as identified by the state in which work is being performed under the contract. All other definitions are as defined in CFR 40 Subchapter E, Parts 162-180.

C. SCHEDULING

The Task and Frequency charts of Integrated Pest Management (IPM) tasks are shown in Attachment Four and the indicated time frames shall be the minimum basis for the scheduling of pest management tasks. The Contractor shall provide a work schedule to the COR prior to the contract start date. The work schedule must be approved by the COR before work begins.

Pest management tasks shall be scheduled and performed on definite appointed days and in no event shall the Contractor carry on work outside regular agreed-upon working days or hours without prior approval of the COR. This service shall include scheduled treatments as well as treatment required as a result of VA complaint log register.

In the event climate or weather conditions become unsuitable for work or may induce an environmental hazard, then the work shall be rescheduled to a date and time satisfactory to the Contractor and the COR. Postponement will not reflect any penalties assessed to the Contractor, nor should there be any additional cost to the Government.

The Contractor is specifically required to provide regular recurring treatment to nourishment kitchens, housekeeping aid closets, utility rooms, patient bathroom facilities and other areas throughout the facility where conditions exist that are generally considered breeding grounds for roaches. This does not limit the Contractor to only these specific sites, but indicates these must be included in the treatment schedule along with other areas the Contractor determines as sites for regular treatment.

All properties will be serviced 3X per week with a printed service report provided after each visit. Service information will be recorded using an electronic documentation system that is used to generate onsite, easy-to-read reports.

D. CONTRACTOR COORDINATION OF WORK

The Contractor shall designate a person or persons who shall have the responsibility for coordination of the contract requirements with the Government’s Contracting Officer Representative (COR). The Contractor’s Coordinator shall assure that work schedules and written reports are properly transmitted to appropriate personnel to meet contract requirements.

E. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

The QASP under the proposed contract shall be based upon the timeliness and quality of the services provided. All work shall commence as specified and will proceed to accomplishment without undue delay. Work on this IPM program service will be performed by competent applicators, experienced and qualified to work on the specific tasks and all work performed shall be first class and in accordance with good commercial practice. These measurements are defined as:

1. Timeliness: Required services and reports are provided in accordance with the stated period of performance, scheduled timelines, and hours of operation as set forth within this solicitation package.

2. Quality:

a. Services provided under the proposed contact will be measured based upon the Government’s determination of the validity, accuracy, clarity and usefulness of the service or if the work performance has met the requirements listed as communicated to the contractor by the Government.

b. Quality will be determined by the number and type of complaints, results of random and scheduled inspections, and the quality of reporting as defined within this statement of work.

c. A successful program will be considered satisfactory when pest numbers are being steadily reduced to the point where they are not interfering with operations, destroying property, or adversely affecting health and morale of patients, staff, and guests.

3. Average Response/Resolution to Emergency Calls: The Contractor shall respond (via phone call to COR) to emergencies within one (1) hours after notification by the COR. The Contractor shall report to perform necessary services no more than ONE (1) hour after notification or within a mutually agreed upon time.

F. SERVICE CALL BACK REQUIREMENT

2. Unscheduled Service: The Contractor shall, within 24 hours after receipt of notification by the Manager, EMS or the COR/s; and, at no additional charge to the Government, perform IPM services to correct the condition. Unscheduled visits must be approved by the COR/s.

3. Emergency Service: The Contractor shall, within four (4) hours after receipt of notification by the Manager, EMS or COR/s; and, at no additional charge to the Government, perform IPM services to correct the emergent condition. A pest emergency refers to any sighting that may have an immediate harmful impact to patients or staff, which if not treated immediately could jeopardize the health and safety of patients, staff, and visitors.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

2. FAR 52.212-2 Evaluation-Commercial Products and Commercial Services: In accordance with RFO FAR 12.201-1 and FAR 12.201-2 when appropriate, the contracting officer may (1) Insert the provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, in solicitations for commercial products or commercial services.

In accordance with FAR 12.203(c)(2), the quotes may undergo a comparative evaluation to determine which quotes are the best overall value to the government. (Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government).

The following are the decision factors:

Factor 1 -Technical Capability Factor 2- Past Performance Factor 3 - Price

Technical and past performance, when combined, are approximately equal to price.

Factor 1. Technical Capability - The Government will assess the Contractor’s ability to demonstrate a clear understanding of the services required and its ability to perform the work adequately and satisfactorily in accordance with the Statement of Work and the extent to which potential risks are identified and mitigated, its qualified key personnel, quality control program, and past experience. Having no experience with work of similar nature will be considered a high-risk factor in evaluation.

Factor 2. Past Performance – The Government will assess the relative risks associated with a Contractor’s likelihood of success in fulfilling the solicitation’s requirements as indicated by the past performance records. The Government may also consider other relevant information when evaluating an offeror’s past/present performance obtained from other federal agency web sites, i.e., the Contractor Performance Assessment Reporting System (CPARS), etc. The Government reserves the right to consider past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement provided by the offeror, as well as information obtained from any other sources, to establish a past performance rating. A proposal containing no relevant past performance shall have past performance evaluated as “neutral”.

Factor 3. Price - The Government will evaluate offers for award purposes based on the pricing for all options to the total price for the basic requirement. The proposed price will be analyzed for reasonableness. It may also be analyzed to determine whether it is realistic for the work to be performed, reflects a clear understanding of the requirements and is consistent with the offeror’s technical proposal. Offers for partial requirement will not be evaluated and will be deemed non-responsive upon receipt. Offerors shall ensure that all pricing is calculated accurately. Any errors on pricing calculations may result in rejection of the offer or require correction at no additional cost to the Government.

Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

For purposes of award, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be those rates in effect under the contract each time an option is exercised under this clause. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.

3. Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://sam.gov/.

“System updates may lag policy updates. The System for Award Management (SAM) may continue to require entities to complete representations based on provisions that are not included in agency solicitations. Examples include 52.222-25, Affirmative Action Compliance, and paragraph (d) of 52.212-3, Offeror Representations and Certifications—Commercial Products and Commercial Services. Contracting officers will not consider these representations when making award decisions or enforce requirements. Entities are not required to, nor are they able to, update their entity registration to remove these representations in SAM.”

4. FAR 52.212-4, “Contract Terms and Conditions–Commercial Products and Commercial Services” (NOV 2023)(DEVIATION NOV 2025)
