Item 0001: The licensed operator is to be on site Monday through Friday, 7 hours per day. The way that it is currently structured is for 5 hours a day, 7 days a week. The staff at the Lyons Medical Center requested that the treatment plant be tended to on weekends since the VA personnel is not on site on the weekends. How would you like for this to be handled?
The hours can be adjusted to meet VA needs as stated in SOW 2nd page-. Normal work hours are considered to be 8:00 am to 4:30 pm Monday thru Friday but can be adjusted as necessary. Sign-in sheet will be posted inside the facility with hours worked.
Item 0002: The Waste Characterization Report is an annual report (not monthly as stated) in which only the data from % Total Solids is used to complete this form. Correct. This an Annual Report. Frequency of this Line item should reflect once a year.
Item 0003: The Residuals DMR parameters do not need to be conducted at the same time as the Chronic Toxicity testing, but the Surface Water WCR parameters do. Correct. Remove Residual DMR Parameters to be conducted at the same time as Chronic Toxicity testing and include Surface Water WCR parameters to be tested at the same time as Chronic Toxicity testing.
Item 0005AA: Ammonia Nitrogen should be in mg/l, not kg/d. This is correct. Make change from kg/d to mg/l.
Item 0005AC: 72-hour turnaround is not the typical turnaround time for a lab. Typical turnaround time is 7-10 business days. This is acceptable for this contract. Will accept this time frame

Item 0006: Semiannual periods are inaccurate. Should be 4/1 - 9/30 and 10/1 - 3/31. This is correct. The Semi-annual periods should be 4/1-9/30 and 10/1-3/31
Item 0007: Pesticides and Acids are not required. Selenium, Thallium, Beryllium and Barium are required, which is not mentioned. Correct. Add Selenium, Thallium, Beryllium, and Barium testing to IGCE. Remove Pesticides and Acids
Item 0008: The parameters mentioned are not required on an annual basis, but are required once per permit cycle, which is 4/1/28 to 9/30/28. This is Correct. Line item 0008 should be removed the base year, option year 1, option year 4 and only added to Option year 3, as per permit No. NJ0021083
Item 0009: Not sure what is meant by 72-hour turnaround for completion of Surface Water Discharge Monitoring Report. DMR's are due by the 25th of the following month of testing. Submit a monthly DMR within 25 days after the beginning of every month. Beginning of the effective date of the permit. As referenced in Lyons VANJHCS Permit No. NJ0021083. Please omit the 72 hours turn around period.
Item 0010: Fecal Coliform is not required. Correct. E.Coli is required per our permit of twice a month for a grab sample. Lyons VANJHCS Permit No. NJ0021083

The aforementioned items also pertain to the subsequent years after the first year of the contract. These corrections will also apply to the subsequent option years.
There is no mention of the monthly Residuals Transfer Report that needs to be submitted to NJDEP. This is a requirement and will be added to a line item.
There is no mention of the parameters needed for testing for the Surface Water Waste Characterization Report. These parameters are Nickel, Silver, Cadmium, Lead, Chromium, Antimony, Mercury & Cyanide. The LC50 test needs to be performed simultaneously with these tests. Nickel, Silver, Cadium, Lead, Chromium, and Antimony must be a 6-hour composite sample. Mercury and Cyanide are grab samples per our Lyons VANJHCS Permit No. NJ0021083
Lastly, does the licensed operator of the facility need to be present for all 35 hours of the week? In the event of the LO being out due to sickness, tenting to family matters, vacation, etc., can someone from the company be present at the facility for coverage? Contractor is responsible for all coverage of 35 hours and must be responsible for any time they are not able to cover.
