About this file

This is a Statement of Work (SOW) for the procurement of a HD03 Hemodialysis Flow System replacement for the Albany Stratton VA Medical Center. The VA intends to award a sole source, firm fixed price contract to Transonic Systems Inc. for one HD03 Monitor, one H4FX Sensor, and one Patient-less Measurement Module (PMM) system configuration. The equipment is designed to measure true delivered flow using transit-time ultrasound technology, blood recirculation, and access blood flow through saline injection and indicator dilution flow measurement technology. The system will replace existing equipment that is over 10 years old and at end of life. The standalone HD03 device operates with two sensors that measure based on existing dialysis machine arterial/venous lines without requiring additional IV lines or integration into the dialysis machine.

The monitor is intended for trained medical personnel to monitor hemodialysis patients and measure three key diagnostic parameters: access recirculation (to verify proper needle placement and pump settings), access flow (to monitor patency of the access device), and delivered blood flow (to confirm the patient receives prescribed treatment). Patient information is stored locally on the device and not connected to VA or contractor networks. Delivery is required to Albany VA Medical Center at 113 Holland Avenue, Albany, NY 12208, with James.Jerian@va.gov designated as the warehouse point of contact. Inservice training will be required as scheduled with the point of contact. Responses to the notice of intent to sole source are due by February 2, 2026, at 4:00 PM Eastern Time and should be submitted via email to Ryan.Seburn@va.gov.

Statement of Work HD03 Hemodialysis Flow System replacement 1/08/2026

A. GENERAL INFORMATION

1. Title of Project: HD03 Hemodialysis Flow System replacement

2. Type of Contract Contemplated: Firm Fixed Price equipment procurement (Brand Name Only Limiting Sources)

The Albany Stratton VA is in need of a new HD03 Hemodialysis flow system intended to measure true Delivered Flow using transit-time ultrasound technology, Blood Recirculation and Access Blood Flow through a simple saline injection via indicator dilution flow measurement technology. This is to replace a flow system that is 10+ years old and is at end of life.

The flow system is for use in patients undergoing Hemodialysis treatment with a catheter (for Delivered Flow and Recirculation measurements), or a graft or fistula (for Delivered Flow, Recirculation and Access Flow measurements)

Patient Information is stored locally on device, not connected to VA network or contractor devices/network.

Must have a monitor with following capabilities:

Monitor is intended for use by trained medical personnel on patients receiving hemodialysis treatment, for the routine monitoring of the following patient and blood handling diagnostic parameters:

• Access Recirculation: A check on proper dialysis needle placement and dialysis pump setting in relation to flow in the access. Access recirculation would prevent the patient of receiving its full hemodialysis prescription. Access recirculation that cannot be mitigated by altering needle placement should be referred to the attending physician, for a change in the patient hemodialysis prescription and as a possible indication of deteriorating flow in the access device.

• Access Flow: Allows the routine monitoring of patency of the access device. A substantial decrease in flow passing through the graft or fistula from the initial level may indicate deterioration of the access, and should be referred to the attending physician for further diagnostic assessment and treatment.

• Delivered Blood Flow: A check on the hemodialysis pump setting, to confirm that the patient receives the hemodialysis treatment prescribed by the attending physician.

Compatibility with Existing Equipment: Standalone HD03 device that operates with 2 sensors that measure based on the existing dialysis machine arterial/venous lines so no additional IV lines need to be inserted into the patient. Given that the device is standalone it also wouldn’t need to be installed and integrated into a dialysis machine.

Training: Inservice training required as scheduled with POC:
