About this file

This is a Quality Assurance Surveillance Plan (QASP) and Monthly Performance Report template for a courier service contract. The QASP establishes systematic methods for evaluating the performance of the courier service provider to ensure compliance with contract requirements and specifies five key performance metrics: Timeliness of Deliveries (95% AQL with 2% penalty per 1% below standard), Accuracy of Deliveries (100% AQL with 5% penalty per 1% below standard), Condition of Deliveries (100% AQL with 5% penalty per 1% below standard), Customer Service (95% positive feedback with 1% penalty per 1% below standard), and Compliance with Safety and Security Protocols (100% AQL with 5% penalty per 1% below standard). Surveillance methods include direct observation, inspection of delivered items, documentation review, customer feedback collection through surveys and complaint logs, and formal audits of records and processes. The Contracting Officer's Representative (COR) or Contracting Officer (CO) will conduct weekly reviews of timeliness, monthly audits of accuracy and condition, monthly customer surveys, and random spot checks for safety compliance.

The accompanying monthly performance report template provides a standardized format for documenting contractor performance against established standards, calculating invoice reductions for non-compliance, and identifying corrective actions. Penalties are applied directly to monthly invoices based on the variance between actual and required performance levels. The document emphasizes contractor accountability with opportunities to correct deficiencies, while persistent non-compliance may result in additional penalties up to contract termination. Performance assessment results are shared monthly with both the contractor and contracting office to ensure transparency, and the QASP will be regularly reviewed and updated based on feedback from the contractor, customers, and stakeholders to support continuous improvement. This template is designed specifically for the VISN1 Lab/Pathology/Pharmacy Courier Service solicitation (36C24126Q0174) for the Department of Veterans Affairs.
