1.0 INTRODUCTION & EXECUTIVE SUMMARY

Mabbett and Associates, Inc. (Mabbett®), performed a follow-up and pre-abatement design survey for suspect asbestos containing building materials (ACM) of Building 20 at the Northampton VA Medical Center (VAMC) located at 421 North Main Street, Leeds, Massachusetts, under VA Contract Number VA241- 15-D-0176, and VA Project Number 631-16-040. The purpose of this survey was to review and confirm the results of the previously existing asbestos survey reports prepared by Mabbett in 2010 and ATC in 2016 and to collect supplemental bulk samples of previously unidentified suspect materials such that a Traditional Asbestos Abatement Plan per the VA Master Specifications dated 1 September 2015 could be prepared in preparation for the complete renovation of Building 20.

The supplemental site survey was performed 1 June 2016 by appropriately credentialed Mabbett personnel as required. Collected suspect ACM samples were submitted to a certified analytical laboratory for asbestos analysis by polarized light microscopy (PLM) using positive stop methodology where indicated. The survey effort involved the collection of the following samples, resulting in the following conclusions:

• 28 additional bulk samples for suspect ACM were collected from Building 20;

• Laboratory analysis of the additional suspect ACM samples collected revealed that none contained asbestos greater than or equal to 1%; and

• The sprinkler pipe thread sealant (throughout the building), contains trace asbestos

(greater than 0% but less than 1%).

This survey report consists of a summary of findings, floor plans indicating positive sample locations, detailed analytical findings for the specific surveyed building materials, and photos of the previously Mabbett identified ACM. Laboratory analytical reports for this building report are located in Appendix A.

2.0 BUILDING DESCRIPTION

According to information provided by the VAMC Office of Facilities Management and observations made by the Mabbett team at the time of the supplemental survey, Building 20 is a three-story vacant Administrative building built in 1923 and occupies approximately 20,784 square feet. The VA Plans to completely abate the ACM throughout the building prior to a full-scale renovation project.

3.0 ASBESTOS SURVEY

3.1 Sampling Methodology

A visual screening inspection was conducted by state licensed asbestos inspectors throughout the building to identify locations of suspect ACM. Only areas that were accessible during the field work phase were inspected. Every effort was made during the supplemental site survey work to access areas as necessary to complete the survey.

However, if any areas remained inaccessible to the survey team, they are indicated in Table 1 below.

Bulk samples were collected of suspect ACM in accordance with the U.S. Environmental Protection Agency (U.S. EPA) Asbestos Hazard and Emergency Response Act (AHERA), the National Emission Standards for Hazardous Air Pollutants (NESHAP), and the VISN 1 approved Mabbett VISN 1 Survey Program Standard Operating Procedure (SOP).

Roofing materials were not sampled to maintain applicable warranties. Bulk suspect ACM sampling of previously identified suspect materials was conducted according to the following sampling plan:

(a) Surfacing material:

(1) At least three bulk samples shall be collected from each homogeneous area that is 1,000 ft2 or less;

(2) At least five bulk samples shall be collected from each homogeneous area that is greater than 1,000 ft2 but less than or equal to 5,000 ft2; and

(3) At least seven bulk samples shall be collected from each homogeneous area that is greater than 5,000 ft2.

(b) Thermal system insulation:

(1) At least three bulk samples shall be collected from each homogeneous area of thermal system insulation;

(2) At least one bulk sample shall be collected from each homogeneous area of patched area of thermal system insulation;

(3) Sufficient samples shall be collected from elbows and fittings to determine if it contains ACM;

and

(4) Bulk samples shall not be collected from any homogeneous area where the state licensed asbestos inspector determined that the thermal system insulation is fiberglass, foam glass, rubber, or other non-ACM.

(c) Miscellaneous material:

(1) At least one bulk sample shall be collected from each homogeneous area that is less than 100 ft2; and

(2) At least three bulk samples shall be collected from each homogeneous area that is greater than

100 ft2.

3.2 Analytical Methodology

The collected bulk samples were submitted under chain of custody procedures to ProScience Analytical Services, Inc. (ProScience - MA ID# AA000156) of Woburn, MA for Polarized Light Microscopy (PLM) analysis of bulk materials via EPA 600/R-93/116 Method.

Sample results from the Mabbett 2010 survey that revealed trace concentrations of asbestos by PLM were re-analyzed using transmission electron microscopy (TEM) analysis.

ProScience is accredited by the American Industrial Hygiene Association (AIHA) and participates in the National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 200090-0).

Bulk sample results are summarized in Tables 2, 3, and 4 – Summary of ACM Building Survey Results. The EPA and Occupational Safety and Health Administration (OSHA), and the Commonwealth of Massachusetts department of Labor Standards (MA DLS) define any material that contains greater than or equal to one percent (≥1%) asbestos as being an ACM. The Commonwealth of Massachusetts Department of Environmental Protection (MA DEP) defines any material that contains any amount of asbestos, less than one percent (<1%) trace asbestos as being an Asbestos Containing Waste Material (ACWM) and should be disposed of as ACWM. As such, the analytical laboratory identified bulk samples as positive for asbestos that met the regulatory criteria of equal to or greater than one percent (≥1%) asbestos and trace for asbestos that met the regulatory criteria of less than one percent (<1%) asbestos.

3.3 Summary of Asbestos Containing Materials (ACM) Findings

3.3.1 Supplemental Site Survey – Mabbett - June 2016

Mabbett Associates, Inc. inspectors Mr. Kevin Donovan and Mr. Darren Andrews, who are EPA certified and Massachusetts Department of Labor Standards (MA DLS) licensed asbestos inspectors (MA DLS # AI002014 and #AI000294, respectively), conducted a supplemental site survey on 1 June 2016. The purpose of this survey was to review and confirm the results of the previously existing asbestos survey reports prepared by Mabbett in 2010 and ATC in 2016 and to collect supplemental bulk samples of previously unidentified suspect materials. In addition Mabbett conducted a room by room inventory of all ACBMs and video taped the exterior and interior of the building. Mabbett collected a total of twenty eight (28) samples and delivered them to ProScience for PLM analysis via EPA 600/- 93/116 Method. Two (2) samples of sprinkler pipe thread sealant contained trace amounts (<1%) of chrysotile asbestos. This material is assumed present throughout the building in

Northampton VAMC - Building 20 S:\ADMINA\7062001 VISN1 AE Contract\TO1 Northampton\Asbestos Survey Report\Asbestos Survey Rpt Northampton Bldg 20.doc VA Project No. 631-16-040 9 June 2016 ©2016, Mabbett & Associates, Inc. Page 4 of 5 various pipe chases and within walls.

A summary all positive ACM analytical results can be seen on Table 5 and the laboratory analytical reports can be seen in Appendix A.

3.3.2 Asbestos Inspection - Survey – ATC – April 2016

ATC Group Services LLC (ATC) inspector Mr. Edward Kolodziej, an EPA-trained and Massachusetts Certified Asbestos Inspector (MA DLS #AI073072), conducted an asbestos survey on 20 March 2016. ATC performed a limited pre-renovation survey to identify suspect asbestos-containing materials in building #20. The survey included a visual assessment of accessible suspect asbestos-containing materials and subsequent sampling and analysis. ATC collected forty three (43) samples and transported under chain of custody to EMSL Analytical, Inc. located in New York, NY for Polarized Light Microscopy (PLM) analysis (EPA Method 600/R-93/116). Analytical results indicated that one (1) sample (B20-14A) of leveler/black mastic contained 4% chrysotile asbestos. This sample was collected from Room 125 and is assumed throughout the building.

A summary all positive ACM analytical results can be seen on Table 5 and the laboratory analytical reports can be seen in Appendix A.

3.3.3 Asbestos Containing Materials Report - Mabbett - December 2010

Mabbett MA DLS licensed asbestos inspectors conducted building surveys between May and June 2010; this included Building 20. The purpose of the survey was to identify locations, collect samples of accessible suspect materials, photograph suspect materials and conduct a lead based paint assessment. Mabbett collected a total of one hundred and sixty four (164) bulk samples and sent them to ProScience for PLM analysis and TEM analysis. Seventy five (75) samples contained asbestos. These materials included; floor tile and mastic, pipe insulation and pipe fitting insulation, and fire doors which are found throughout the building. One-foot by one-foot ceiling tiles with glue dabs are found in the basement stairwell and second floor hallway. Door frame/header caulking and window frame/header caulking, were are found on the exterior of the building.

A summary all positive ACM analytical results can be seen on Table 5 and the laboratory analytical reports can be seen in Appendix A.

3.3.4 CADD Drawings

The location of each collected bulk sample and its abridged sample ID is shown on the CADD drawings in the Figures Section of this report. The sample ID on the drawings have been abridged (by excluding the individual room number from the full sample ID) for aesthetic purposes. Each positive ACM sample location is colored red. Building areas containing ACM have been indicated with hatching to identify the location of the identified

ACM. The CADD plans reflect the results from all three surveys; Mabbett 2016, ATC 2016, and Mabbett 2010.

4.0 LIMITATIONS

This inspection report is the result of a diligent search of Building 20 for ACM. Potential suspect ACM may exist in inaccessible areas of the building that could not be assessed during this survey as described in Section 3.1 above. Mabbett conducted this assessment with reasonable care and has performed this project within generally accepted industry standards. Mabbett recommends that abatement contractors provide unit pricing for typical materials in case unforeseen ACM is encountered during the abatement project. There can be no assurances, and Mabbett makes no assurances, that the information, research, and technology used to prepare this report may not change in the future, thus affecting the results provided.

5.0 CLOSING REMARKS

Upon completion of a review of readily available historical asbestos survey documents (Mabbett 2010 and ATC 2016) and upon completion of a supplemental asbestos survey Mabbett identified numerous ACMs throughout Building 20 located on the Northampton VA campus.

1. All ACMs located in Building 20 are required to be properly abated prior to any renovation activities in accordance with the U.S. EPA’s NESHAP, MA DEP, and MA DLS regulations.

2. All asbestos abatement work must be performed by a Commonwealth of Massachusetts licensed Asbestos Abatement Contractor and must be performed in accordance with all federal, state, and local regulations.

3. Any suspect ACM identified during the course of the abatement or renovation activities must be assumed to be ACM until additional bulk sampling is performed.

4. Mabbett will provide the Northampton VAMC with a Traditional Asbestos Abatement Specifications report written in accordance with the VA’s Master Specifications dated 1 September 2015.

* - The VISN 1 hazard assessment scale 1 – 4 is a relative indicator of the risk and need for response/remediation. (1) represents the highest priority (e.g. removal or encapsulation) whereas a (4) represents the lowest priority (monitor as part of 6 month O&M program). The rating, assigned by an Asbestos Management Planner, takes into account the following criteria:

condition, friable vs. non-friable, accessibility, occupancy (e.g. continuous, intermittent or occasional, patients/staff/visitors), potential for air erosion, potential for vibration damage, potential for disturbance/damage (e.g. exposed and in an accessible location), and potential for water damage.
