A.2 CALCULATION OF SELF-PERFORMED WORK
SUBMITTED IN RESPONSE TO RFP VA241-17-R-0505 Emergency Rebuild 1st Floor B and C Wing, VA Boston Jamaica Plain Campus.
Use a format similar to the following to identify and calculate cost of the work to be self-performed. Refer to the definitions pertaining to “Self-performance of work”, “On the Site” and “Total amount of work to be performed under the contract”.
(Includes mobilization and utilization of owned or rented plant and equipment to be operated by the prime contractor’s own employees; only those materials which will be both purchased and installed by the prime’s own forces; labor associated with those aforementioned materials or equipment; only those supplies to directly support work performed by the contractor’s own employees; and the contractor’s own job overhead costs.)
Clearly describe the work to be self-performed:
Show Calculation of Self-Performed work: __________________________________
B.1) Total Offer Price: $_________________
B.2) If applying for consideration as a General Contractor (multi-discipline projects) work must be self-performed $_________________
B.3) Subtract G&A, home office overhead, prime contractor’s markups for profit, bond, state use tax, etc. $_________________
B.4) Remainder is “Total amount of work to be performed under the Contract” = $_________________
B.5) “Work to be self-performed”: = $_________________ Amount shown on this line should match the amount shown for “Show Calculation of Self-Performed work”?
B.6) 15 % Self-performed Work = Line B.5 / B.4 X 100% = ________________________%

INFORMATION REGARDING BIDDING MATERIAL, BID GUARANTEE AND BONDS
1.1 SCOPE OF WORK
RENOVATE SPACE FOR THE RELOCATION OF INPATIENT PHARMACY
The project is located in Wing C of the 2nd floor of the main hospital, Building 1, on the campus of the Providence VA Medical Center located at 830 Chalkstone Avenue, Providence, RI. Building 1 is a nine story concrete framed structure constructed in 1948 and opened to patients in 1949. The hospital provides both inpatient and outpatient services to military veterans that live in Rhode Island and southeastern Massachusetts.
DESCRIPTION:
The new pharmacy will be located in the existing Clinical Engineering area on the second floor of Wing C, Building 1. The purpose of the project is to relocate the existing Inpatient Pharmacy to an area that meets current codes, with a layout that reflects current pharmacy workflow and the needs of the hospital. The new space will meet new United States Pharmacopeia (USP) 800 regulations, meet VA and industry healthcare standards and meet VA security requirements.
Work involves selective demolition of the existing Clinical Engineering area with remediation of hazardous materials. The demolition work will entail removing interior partitions, doors and frames, flooring and ceilings. The new design provides quieter work areas for pharmacists, a better equipment layout for pharmacy technicians, appropriate storage needs and sterile and hazardous preparation rooms within the pharmacy suite. The project accommodates an equipment layout in the pharmacy work area provided by TCGRx, the vendor for specialized equipment to be purchased by the VA. Work includes, but is not limited to asbestos abatement, demolition, removal of existing structures, furnishing of labor and materials, supervision, equipment, walls, ceiling, rough and finish carpentry, flooring, fire protection, plumbing, HVAC including dedicated exhaust systems, Electrical, and other items of work.
BASE OFFER - GENERAL CONSTRUCTION:
A. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures/equipment, and furnish labor and materials and perform work for Pharmacy Renovations Building 1, VAMC 830 Chalkstone Avenue, Providence, RI.
B. Work includes necessary supervision, labor, equipment, material and infrastructure required to complete work, necessary removal of existing structures and construction and certain other items, asbestos abatement, demolition, removal of existing structures, general construction, alterations, construction of walls, pharmacy vault, ceiling, rough and finish carpentry, casework, flooring and other interior finishes, fire protection, plumbing, HVAC, Electrical, Security, and other items of work.
C. Notice to proceed through Construction phase duration is not to exceed 210 calendar days.
PERIOD OF PERFORMANCE: The entire scope of work ready for use, including final cleanup of the premises and submission of close out documents, shall be completed not later than 210 calendar days after contractor receipt of Notice to Proceed from the Contracting Officer. Period of Performance includes 90 calendar days for submittals. All submittals shall be complete and submitted within 90 calendar days after Notice to Proceed.
PROPRIETARY ITEMS: Notwithstanding any other provision of the contract, no other material, equipment, testing, installation or product of the following proprietary items will be acceptable for use as a substitution:
1) Fire Alarm System – Notifier System by FSI Justification: No substitutions. Sole Source documentation to be submitted separately as needed.
2) Direct Digital HVAC Building Management Control System (Metasys) and Security System Pegasys – by JCI (Johnson Controls, Inc.). Justification: No substitutions. Sole Source documentation to be submitted separately as needed.
3) Keying, Door Locks and latches - PVAMC Sargent Keso Grand Master Key System. Justification: Cylinders for all locksets shall be keyed into existing PVAMC Sargent Keso and have removable core to match existing VAMC Providence core system. Items are the facilities standards. The Providence campus has all locks and keying under this Master System for the purpose of ensuring security of the site , ease of access for authorized key holders, and ease of maintenance of the keying/lock system by our locksmith wo has an inventory of appropriate door locks and spare parts; it would be uneconomical for VA locksmith to create additional inventories of other manufacturer’s door locks. Therefore, only this manufacturer’s products are acceptable; no substitutions.
