About this file
This document includes a past performance questionnaire for a federal solicitation and details of the related opportunity. The solicitation seeks industry partner support to design a robust, leading suicide prevention program for Veterans that considers their perspectives and outlines potential barriers. It aims to reduce military and Veteran suicide through early intervention, mental healthcare access, and suicide prevention services. The past performance questionnaire assesses offerors on quality, schedule, cost control, management, small business utilization, regulatory compliance, and an overall recommendation. Respondents are asked to evaluate a vendor's prior contract performance in these areas on a scale from exceptional to unsatisfactory.
