Q.1What is the brand that is currently in use?
A.1The current contract is VA797N14C0033. Also, public information is available at usaspending.gov.
Q.2Pricing Spreadsheet: Is Oversize tube a separate line item or is the price to be included in each system?
A.2Yes, this is a separate line item which should be priced separately and not included with the system.
Q.3Pricing Spreadsheet: Is each contract year to be priced separately on this one spreadsheet?
A.3The pricing sheet would be allocated to each ordering period. An ordering period would be equivalent to 12 months.
Q.4Pricing Spreadsheet: Can you define SLA fee and is it supposed to be reflected in our price quote?
A.4The SLA fee is a Service Level Agreement Fee which is a 3% fee added to the unit cost. As stated in the solicitation, this fee will be remitted to the SAC. This will not be included in the evaluated price.
Q.5Pricing Spreadsheet: What are the quantities required on the bid sheet?
A.5This contract will be awarded as an Indefinite Delivery Indefinite Quantities (IDIQ) contract, quantities are not specified. However, we did provide a parameter that can be expected. The total value of the contract will not exceed the maximum ceiling price.

Q.6 Section 1.4.1 – g: You are asking for a Patient Unique Identifier on the invoice. Will we be receiving this on every PO now or will this not be required?

A.6 All purchase orders from prosthetics have the vets name, address, PO number, and phone number. For stock orders that will be shipped to the VA, this will include the veteran’s name, the purchase order number, and the address of the VA

Section 1.4.1 has been updated to remove the Patient Unique Identifier.

Q.7Section 2.4 – a: An oversized/Large Penile Tube must now be included with every unit? Is this an error as on the Price Cost Schedule you have the item list as CLIN 1004, 2004, 3004, 4004, & 5004?
A.7The Oversized/Large Penile Tubes would be purchased as a separate item and priced out as a separate item. However, vendors shall have the capability to have an oversized/large Penile Tube as a part of the system.
