Instructions

RATE SHEET TAB INSTRUCTIONS:

1. Enter Offeror Name in cell C1.

2. Enter Handling rates in Years 1 through 5 for Travel and Materials in red cells in rows 5 and 6. The proposed Travel Handling and Materials Handling rates will be incorporated into any resultant contract as Not-To-Exceed (NTE) ceiling rates.

3. Enter fully loaded blended labor rates in red cells for Years 1 through 5 in rows 14 thru 98. Labor rates shall be no more than two decimal places. Labor rates may be escalated from year to year, however the highest proposed fully loaded blended labor rate for all five years (presumably Year 5) shall be at or below the maximum fair and reasonable labor rates listed in column I. The proposed fully loaded blended labor rates will be incorporated into any resultant contract as Not-To-Exceed (NTE) ceiling rates. Awarded ceiling rates will apply to FFP and T&M/LH orders. However, at the task order level for FFP orders, Offerors may propose other labor categories when a particular labor category is not already on contract. VA expects this to be the exception rather than the rule and that awardees will be required to adhere to their proposed labor categories.

4. If cells are not correctly filled out, cells are formatted to be flagged and will be color shaded. Ensure all cells are populated so no color shading is remaining in the Offeror's Name cell, Handling Rates cells, and Fully Loaded Blended Labor Rates cells before submitting the proposal.
