About this file

This document is a Vendor's Statement form from the City of Owensboro, Kentucky, pursuant to Kentucky Revised Statute (KRS) 45A.343. The form requires vendors to disclose and certify compliance with specific state tax and labor regulations covering corporate taxes, sales and use taxes, income taxes, wage and hour standards, occupational safety, unemployment, and workers' compensation. The document serves as a mandatory declaration for potential contractors, affirming they have not been found in violation of these statutory provisions within the past five years and committing to ongoing compliance throughout the contract period.

The statement outlines potential consequences for nondisclosure or noncompliance, which include contract cancellation by the City of Owensboro and disqualification from future city contracts for two years. The form is designed to be completed by a duly authorized company officer and requires the vendor to acknowledge their understanding of these requirements. While no specific contract details are provided, the document represents a standard compliance mechanism for local government procurement, ensuring vendor accountability and adherence to state regulations across multiple statutory domains.
