About this file
This is a Vendor's Statement form pursuant to Kentucky Revised Statute 45A.343 required by the City of Owensboro, Kentucky for contractors seeking to do business with the local public agency. The document serves as a compliance certification form that vendors must complete and submit as part of their contracting process, though it does not specify particular products, services, quantities, or contract terms as it is a standardized legal compliance form applicable to all city contracts.
The form requires vendors to certify they have not violated specific Kentucky statutes within the preceding five years, including corporate taxes (KRS 136), sales and use taxes (KRS 139), income taxes (KRS 141), wage and hour laws (KRS 337), occupational safety regulations (KRS 338), unemployment provisions (KRS 341), and workers' compensation requirements (KRS 342). Vendors must acknowledge their ongoing compliance obligation throughout the contract duration and understand that failure to disclose violations or maintain compliance can result in contract cancellation and a two-year disqualification from future city contracting opportunities. The form includes signature fields for company authorization and is dated 2025, with contact information for the City of Owensboro Purchasing Department.
