The form serves as a prerequisite for determining bidder responsibility and requires certification that neither the signatory nor anyone acting on behalf of the bidder or offeror has knowingly violated Kentucky's campaign finance laws. The document includes spaces for completion in 2025 and requires the company name, authorized signatory information, signature, and title of the person executing the statement.

The statement operates under penalty of perjury and specifically defines "knowingly" as being aware or should have been aware that conduct violates campaign finance provisions or that prohibited circumstances exist. The City of Owensboro's Purchasing Department contact information is provided (270-687-8431, 101 E. 4th Street, P.O.
