Vendor Responsibility Questionnaire Summary
This document is a Vendor Responsibility Questionnaire (VRQ) template established by the State of Georgia to evaluate vendor integrity and procurement risk for formal solicitations exceeding $100,000 and/or involving public health, infrastructure, or operational service delivery. The VRQ is grounded in multiple Georgia statutes (O.C.G.A. 50-5-50, 50-5-82, 50-5-84, 84.1, 84.2, and 13-10-91) that mandate governmental entities assess bidder responsibility, including performance history, legal compliance, and prohibition of contracts with debarred vendors, those with unpaid taxes, companies engaged in prohibited activities, or those unable to meet E-Verify requirements for public works projects. All vendors must complete the questionnaire truthfully and comprehensively, providing detailed explanations and supporting documentation for any affirmative responses to legal and regulatory compliance questions, with particular attention to issues such as prior debarment, environmental violations, OSHA citations, procurement fraud convictions, and ongoing investigations that could materially impact performance.
The VRQ requires vendors to provide complete Section A identification information including legal business name, physical address, contact details, Federal Employer Identification Number, state of incorporation, organizational type, and any applicable Georgia professional licenses. Section B addresses legal and regulatory compliance across twelve compliance areas requiring yes or no responses with narrative explanations. Sections C through E address financial and operational stability, tax compliance with the Georgia Department of Revenue, and certification requirements. The final page must be notarized with a valid commission expiration date. Vendors identified as subsidiaries or affiliates must include parent company disclosures. This standardized form serves as a prerequisite screening mechanism to ensure vendor eligibility and reduce procurement risk across Georgia state and local government entities.
