About this file

This is a Vendor Questionnaire for Broward County, Florida, related to a one-time purchase of a pool pump for the Parks and Recreation Division. The solicitation requires vendors to submit responses electronically through Bonfire BPRO by the specified closing date and time. Shipping and delivery must be included in the unit price. Vendors must submit the completed questionnaire with their solicitation response or within three business days of the County's written request; failure to submit timely may result in the vendor being deemed nonresponsive. All questions and clarification inquiries must be submitted through Bonfire BPRO by the designated questions due date, with responses provided in the Bonfire BPRO Messages section.

The questionnaire requires vendors to provide standard business information including legal name, FEIN, office location, contact details, and authorized contacts. Vendors must certify they have not been debarred or suspended by any government entity within the last three years, confirm current manufacturer authorization and warranty terms, and affirm willingness to extend the same prices and terms to other governmental entities during the contract period. Additionally, vendors must certify acceptance of Visa credit card payment without additional fees, disclose any potential conflicts of interest with Broward County officers or employees, confirm non-collusion and independent submission, and certify non-participation in solicitation development that would provide unfair advantage. Vendors may optionally certify implementation of a drug-free workplace program to receive preference in award if submissions are tied. All information must be certified as true and correct by an authorized representative, with acknowledgment that inaccurate statements may result in rejection, award rescission, contract termination, or vendor debarment.
