About this file
This is a Statement of Work for the delivery of Acumed surgical kits to the Navajo Area Indian Health Service's Chinole Comprehensive Healthcare Facility (CCHCF) Operating Room Department in Chinle, Arizona. The contractor shall provide surgery-related Acumed kits on an as-needed basis for foot, ankle, and lower leg fracture fixation procedures, with an approximate annual usage of $50,000. The period of performance extends from the date of award through December 31, 2026, with obligations made as funds become available.
The kits must include instrumentation (tools) and implants of varying sizes, and the contractor shall invoice only for used supplies—specifically implants, pins, guide wire, and single-use surgical items. Unused instruments and trays are not billable. The contractor must deliver kits within 1-2 days, coordinate with the government for timely pickups of returned supplies, and cover all shipping costs for both deliveries and returns via FedEx at no government expense. The government will place orders approximately one week in advance of scheduled surgeries and must return unused kit supplies within 4 business days accompanied by a completed loaner usage form. The contractor shall provide order confirmations and packing slips for all transactions. Performance standards require stellar customer service, experience serving the federal government or large commercial entities within the past three years, and reference letters from entities worked with during that period. Gloria Fulton, CCHCF Operating Room Supervisory Clinical Nurse, or her designee will inspect and accept delivered kits.
