About this file
This is a Statement of Work (SOW) document for the Department of Health and Human Services (DHHS) in Utah detailing technical support and maintenance services for hardware equipment. The contractor shall provide comprehensive support services including a two-hour callback response time, service center repair and on-site hardware troubleshooting within 48 hours of request, priority technical remote support, materials and parts as outlined in the Statement of Coverage (Attachment D), and firmware updates. The Department will conduct meetings with the Contractor throughout service delivery to ensure equipment functions properly, and the Contractor must provide service reports promptly upon work completion. The contract term begins January 1st, 2024, with invoicing to occur at the beginning of each covered year.
Billing shall be conducted annually based on amounts specified in the Quote attachment (Attachment C), with invoices submitted to DHHS at the beginning of each contract year. The related contract opportunity indicates this is a non-competitive award being processed under the Utah Procurement Code without engaging in a standard procurement process, suggesting either a single source justification, consideration of transitional costs, or circumstances that make competitive procurement impractical and not in the best interest of the procurement unit. Specific pricing amounts, funding sources, and detailed justification for the sole-source or non-competitive award are contained in supporting documentation referenced in the contract opportunity files.
