About this file

This is a Disclosure of Lobbying Activities form (SF-LLL) prepared by the Savannah-Chatham County Public School System (SCCPSS) in Georgia for Bid 26-08 regarding a School Nutrition Program - Cleaning Products Annual Contract. The form serves as a disclosure document pursuant to 31 U.S.C. 1352 and is required when federal funds are involved in covered Federal actions. The document provides the template and instructions for reporting entities, whether prime contractors or sub-awardees, to disclose any lobbying activities undertaken to influence the outcome of this covered Federal action. The form requires completion at the initiation of a covered Federal action or upon material changes to previous filings, and must identify the type of Federal action (contract, grant, cooperative agreement, loan, loan guarantee, or loan insurance), the status of the action, the reporting entity classification, and details regarding any lobbying registrants or individuals engaged to influence federal officers, Members of Congress, or congressional staff.

The form emphasizes that filing is mandatory for any payment or agreement to make payment to lobbying entities for influencing the covered Federal action. Failure to file the required disclosure subjects violators to civil penalties ranging from $10,000 to $100,000 for each violation. The disclosure is a material representation of fact and will be reported to Congress semi-annually and made available for public inspection. The form includes standard sections for entering the federal agency, federal program name or description, CFDA number, federal action number, award amount, lobbying registrant information, and individuals performing services. A certifying official must sign and date the completed form, providing their printed name, title, and telephone number.
