About this file

This document is a required forms and questionnaire package for RFP 26-0152 issued by the City of Fort Worth Purchasing Division for Uniform Scrub Rental and Laundry Services. The RFP seeks proposals for a full-service uniform scrub rental program to service the Code Compliance Department on an as-needed basis. The solicitation requires respondents to provide weekly pickup and delivery services at both Fort Worth Area Code Compliance (FWACC) locations and to service uniforms in accordance with specifications outlined in the RFP documents.

Proposers must submit completed required forms including company information, authorized representative signatures, and three recent customer references demonstrating similar scope and scale of uniform rental and laundering services. The questionnaire requires vendors to disclose their length of time in business, current litigation or criminal investigation status, willingness to participate in cooperative purchasing agreements, and proposed prompt payment discount terms. Additionally, vendors must confirm their capability to provide a fully integrated uniform rental program including both garment provision and laundering services, weekly pickup and delivery capability at both FWACC locations, a dedicated account representative or customer support team, on-site sizing for initial implementation, and specify standard response times for routine and urgent service requests. All submissions must include a completed Form W-9 (Request for Taxpayer Identification Number and Certification) and a Conflict of Interest Questionnaire (Form CIQ) in compliance with Texas Local Government Code Chapter 176, filed no later than seven business days after the vendor becomes aware of any potential conflicts of interest with local government officers.
