About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Incorporated County of Los Alamos Procurement Division in New Mexico for the Health Commons Services solicitation. The form serves as a primary covered transactions certification that must be completed and submitted by all offerors and bidders as part of their proposal package. Offerors must certify that neither they nor any of their principals are currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any federal, state, or local department or agency.

The certification requires offerors to attest that within the three-year period preceding submission, they have not been convicted of or had civil judgments rendered for fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, or offenses indicating lack of business integrity. Additionally, offerors must certify they have not violated contract provisions, failed to perform satisfactorily under prior contracts, violated ethical standards under the County Procurement Code, been criminally or civilly charged with enumerated offenses, are not current or former County employees or immediate family members of County employees or public officials, and have not had public contracts terminated for cause or default within the preceding three year period. Any inability to certify these statements requires written explanation, and falsification of any statement constitutes grounds for proposal rejection or contract award rescission.
