About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division, located at 101 Camino Entrada, Building 3, Los Alamos, NM 87544. The form is a standard certification that requires an offeror or bidder to confirm their eligibility and good standing for participating in a county procurement process by attesting that they have not been debarred, suspended, or involved in any criminal or unethical activities that would disqualify them from contracting with the county.

The certification requires the offeror to declare that neither the company nor its principals have been convicted of fraud, antitrust violations, embezzlement, theft, or other offenses that would indicate a lack of business integrity within the past three years. Additionally, the form mandates that the offeror is not a current or former county employee, not an immediate family member of a county employee or public official, and has not had any public transactions or contracts terminated for cause or default. The document emphasizes that any falsification of statements on the form will result in non-consideration of the proposal or potential rescission of a contract award.
