About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form from the Incorporated County of Los Alamos Procurement Division. The form requires potential bidders or offerors to certify that they meet certain legal and ethical standards for participating in county transactions. Specifically, the certification asks companies to affirm they are not currently debarred, suspended, or excluded from public transactions, have not been convicted of fraud or other criminal offenses in the past three years, and are not current or former county employees or immediate family members of county employees.

The document outlines detailed conditions that would disqualify a bidder, including convictions related to fraud, antitrust violations, embezzlement, theft, and other offenses indicating a lack of business integrity. The form emphasizes that falsifying any statement could result in non-consideration of a proposal or rescission of a contract award. Companies are required to thoroughly disclose any issues that might prevent them from meeting these standards, with the understanding that such disclosures do not automatically disqualify them from consideration for a contract.
