About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the County of Los Alamos Procurement Division in New Mexico for use in the Operating Food Pantry Services contract opportunity. The form serves as a required certification that offerors and bidders must complete and submit as part of their proposal or bid package. Prospective contractors must certify their eligibility to participate in county transactions by confirming they are not debarred, suspended, proposed for debarment, or otherwise ineligible at the federal, state, or local level. The form requires authorized signers to attest to their company's compliance status and provide their name, title, date, and signature on the certification document.

The certification requires offerors to confirm that neither they nor their principals have been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records within the preceding three years; have not violated contract performance provisions or ethical standards; are not presently indicted or criminally charged; are not current or former County employees; are not immediate family members of County employees or officials; and have not had public contracts terminated for cause or default within three years. Offerors unable to certify any statements must provide written explanation with their submission. Falsification of any certification statement constitutes grounds for non-consideration of the proposal or rescission of any contract award. The County reserves the right to thoroughly review certifications, which may not necessarily preclude consideration for award.
