About this file

This is a minimum insurance requirements document for Harris Health, a Texas-based healthcare system, establishing mandatory coverage standards for contractors engaged in capital projects. The document outlines comprehensive insurance requirements that scale based on project value, ranging from projects under $25 million to those exceeding $100 million. Contractors must maintain workers' compensation coverage as required by Texas law, commercial general liability with limits from $1 million to $2 million per occurrence depending on project size, automobile liability coverage for all owned, hired, and non-owned vehicles at $1 million per accident, and umbrella/excess liability coverage ranging from $1 million to $5 million. Professional/errors and omissions liability is required when applicable to the scope of work.

All insurance must be issued by companies licensed in Texas with an A.M. Best rating of at least A-VII, and Harris Health must be named as an additional insured on general liability, automobile, and umbrella policies. Contractors are required to provide unaltered insurance certificates prior to commencing work and must maintain products and completed operations coverage for a minimum of five years after final payment. The document specifies that all insurance policies must waive rights of subrogation against Harris Health, its officers, employees, and agents, and contractors must provide 30 days' written notice of any policy cancellation, material changes, or non-renewal. Failure to maintain required coverage may result in contract termination at Harris Health's sole discretion. The requirements are applicable to the Grease Waste Infrastructure Replacement project at Ben Taub Hospital, estimated at $948,500.
