About this file
This is a minimum insurance requirements document issued by Harris Health for contractors performing work on state and local contract opportunities in Texas. The document establishes mandatory insurance coverage standards that all contractors must maintain throughout the contract term, with specific limits and types of coverage varying based on project budget size. Required insurance types include workers' compensation with employers' liability, commercial general liability, automobile liability, umbrella/excess liability, and professional/errors & omissions liability where applicable. All insurance policies must be issued by companies licensed in Texas with an A.M. Best rating of at least A-VII, and Harris Health must be named as an additional insured on general liability, automobile, and umbrella liability policies. Contractors are required to provide unaltered insurance certificates as evidence of coverage and must notify Harris Health at least 30 days prior to any policy cancellation, material change, or non-renewal, with replacement policies required within 30 days of the effective date.
For the Grease Waste Infrastructure Replacement project at Ben Taub with an estimated budget of $948,500, the applicable insurance requirements fall under the "Projects < $25M" category. Contractors must maintain commercial general liability coverage with a $1,000,000 each occurrence limit and $2,000,000 general aggregate, automobile liability coverage of $1,000,000 per accident, workers' compensation at statutory limits with employers' liability of $1,000,000 per accident and per disease, and umbrella liability with $1,000,000 each occurrence and aggregate limits. Products and completed operations insurance must be maintained for a minimum of five years after final payment, and all subcontractors must provide equivalent insurance coverage. Failure to maintain required insurance coverage may result in contract termination at Harris Health's sole discretion.
