About this file

The document is an Instructions for Completing Form 1295 Certificate of Interested Parties for Fort Bend Independent School District (FBISD), which requires vendors to electronically complete, print, sign, and submit Form 1295 with their proposals, in compliance with House Bill 1295 and Texas Government Code Section 2252.908. The form mandates disclosure of interested parties, defined as persons with a controlling interest in the business entity or those actively participating in contract facilitation, and must be filed electronically with the Texas Ethics Commission.

The specific form provided includes details for an interested party named Peter James from Missouri City, TX, who is identified as having a controlling interest. The form requires completion of sections detailing the business entity, governmental entity (FBISD), contract identification, and an unsworn declaration. Vendors must complete the form electronically on the Texas Ethics Commission's website, print it with a unique certification number, and submit a signed copy, even if no interested parties exist. The document emphasizes that FBISD cannot enter into a contract with a business entity that fails to submit this completed form.
