About this file
This document is a Drug-Free Workplace Affidavit that outlines specific requirements for businesses seeking to bid on or perform contractual services. The affidavit requires firms to implement and maintain a comprehensive drug-free workplace policy that includes publishing statements to employees, informing workers about drug abuse dangers and consequences, providing drug-free workplace statements to employees, and establishing clear protocols for reporting and addressing controlled substance violations.
The document details six key steps that a responding firm must certify, including: notifying employees about prohibited workplace drug-related activities, informing staff about drug abuse dangers and available assistance programs, distributing drug-free workplace statements, mandating employee reporting of drug-related convictions within five days, imposing sanctions or requiring rehabilitation for convicted employees, and demonstrating a good faith effort to maintain a drug-free workplace. While no specific contract, jurisdiction, or monetary values are mentioned, the affidavit appears to be a standard compliance document that businesses must complete when pursuing government or institutional contracts.
