About this file

This document provides instructions and definitions for completing Form 1295, the Certificate of Interested Parties disclosure required by Texas House Bill 1295 for all vendors submitting proposals to Region One Education Service Center. The form must be completed electronically through the Texas Ethics Commission website, printed with a certification number, and submitted with all proposals or contracts. All business entities, regardless of whether they are for-profit or nonprofit organizations, must submit Form 1295 even if no interested parties exist. The submission requirement applies to the Janitorial and Disposable Products procurement, which seeks to establish a pre-qualified vendor list for supplies including cleaning chemicals, disposable gloves and aprons, trash liners, food service products such as trays, plates, cups, and flatware, and related goods and services within the scope of the solicitation.

The Form 1295 disclosure identifies interested parties as persons with controlling interest in the business entity (defined as ownership exceeding ten percent, board membership on entities with ten or fewer members, or officer status in entities with four or fewer officers) and intermediaries who actively facilitate contract negotiations for compensation. Certain contract types are exempt from the disclosure requirement, including contracts with publicly traded business entities, utilities, sponsored research contracts, interagency contracts, and specific health and human services contracts. Vendors should register on the Ethics Commission website, complete the form with the proposal number and contract name, and retain documentation including the certification number for submission with their proposal response.
