About this file

This is a URS Post-Retired Verification Form for Independent Contractors (1099 Employees) required by Jordan School District in Utah. The form requires prospective contractors to disclose whether they have retired from the Utah Retirement Systems (URS) and, if so, to provide their signature and date of completion. The form must be submitted to the Accounting Department along with a W-9. This verification is necessary for the Copper Hills High School Freezer Expansion project, an Invitational For Bid (IFB) issued by Jordan School District in accordance with Utah Code Annotated Chapter 63G-6a and applicable Utah Administrative Rules.

Contractors who are URS retirees receiving monthly benefits face restrictions on re-employment with participating URS employers, including work performed as employees paid through payroll or as independent contractors paid through accounting. Retirees must contact URS's Retirement Benefits Department at 801-366-7770 before beginning work and complete URS Form RTRT-27A (Post-Retirement Employment form). Failure to report re-employment violates Title 49 of the Utah Code and may result in liability to repay URS any overpayment amounts. This verification requirement applies to all independent contractors and 1099 employees bidding on the project.

(business).

If it is discovered that a URS retiree failed to report reemployment to URS on its Post-Retirement

Employment form (Form RTRT-27A) in violation of Title 49, the retiree will be liable to repay URS the amount of any overpayment resulting from the violation to report. See Utah code section 49-

11-1207(2).

You are directed to contact URS's Retirement Benefits Department at 801-366-7770 before beginning work with Jordan District. You will be asked to complete and file a
