About this file

This is a vendor certification form required by Halifax Health, a Florida government entity, for participation in contract opportunities. The document serves as a compliance certification whereby vendors must declare their status regarding various scrutinized company lists and business operations restrictions as mandated by Florida Statutes. Vendors submitting proposals for goods or services to Halifax Health must certify that their company is not listed on the Scrutinized Companies that Boycott Israel List, the Scrutinized Companies with Activities in Sudan List, or the Scrutinized Companies with Activities in the Iran Petroleum Energy Sector List, and that they are not engaged in boycotts of Israel or business operations in Cuba or Syria. The certification applies to all contracts regardless of value, though certain restrictions regarding Sudan, Iran petroleum, Cuba, and Syria apply specifically to contracts valued at one million dollars or more.

The form requires vendors to provide standard identification information including company name, Federal Employer Identification Number (FEIN), authorized representative details, and contact information. Vendors must sign and date the certification under penalty of perjury, understanding that submission of false information may result in civil penalties, attorney's fees, and contract termination at Halifax Health's discretion. For contracts valued at one million dollars or above, additional grounds for termination include placement on the scrutinized companies lists or engagement in prohibited business operations in the specified countries or regions.
