About this file

This is a bidder compliance and qualification attachment document for Jackson County, Missouri, specifically related to Request for Proposal No. 26-004. The document serves as a comprehensive submission package that requires bidders to provide affidavits, acknowledgments of addenda receipt, compliance certifications, and detailed statements of contractor qualifications. The affidavit requires authorized company representatives to certify under oath that no county officials have financial interests in the bidder's proposal, that the bidder has not engaged in collusive practices, and that the bidder complies with federal work authorization requirements and is not employing unauthorized aliens. Bidders must acknowledge receipt of all addenda issued up until the Question Response Deadline of 5:00 pm CST on March 10, 2026, and understand that additional addenda may be issued prior to that date through the Bonfire Portal.

The document mandates that all vendors obtain a Certificate of Compliance from the Jackson County Compliance Review Office prior to bid submission, with applications required five business days before the bid response deadline to ensure processing. Bidders must demonstrate that they are either duly listed and assessed on Jackson County tax rolls without delinquent payments or had no property subject to county taxation as of December 31, 2025, and must attest to compliance with Chapter 6 of the Jackson County Code regarding non-discriminatory practices and equal employment opportunity. The Statement of Contractor's Qualifications requires comprehensive information including business structure, years in operation, number of employees, geographical operating limits, history of completed and current contracts, references, and planned subcontractors. Additionally, bidders may claim Minority-Owned Business (MBE), Woman-Owned Business (WBE), or Small Business status with supporting certification documentation, and may submit exceptions to the specifications and scope of services for the county's consideration.
