About this file
This document is an Attachment 9 - Proposed Subcontractors form for a Request for Proposal (RFP) Solicitation No. BPM006719 issued by the Arizona Department of Administration State Procurement Office. The solicitation is for Statewide Emergency Response Services and Solutions, with AMI Expeditionary Healthcare, LLC as the primary contractor. In this specific attachment, the contractor has indicated that they will not be using any subcontractors and will perform all work with their own personnel.
The document provides a template for listing potential subcontractors, including fields for name, contact information, small business status, work to be performed, percentage of contract work, and quality assurance/quality control measures. While no subcontractors are listed in this instance, the form allows for up to five subcontractors to be documented. The solicitation appears to be for emergency response services, with provisions for the state to request additional information about proposed subcontractors as a precondition of contract award.
