About this file
This is a Debarment and Suspension Certification form for the Cherokee County School District (CCSD) Procurement Services Department in Georgia, issued in connection with an RFP for Student Support Contracted Services. The certification document establishes the requirements for bidders participating in the solicitation process and sets forth the district's commitment to awarding contracts only to responsible contractors in compliance with Federal Acquisition Regulation (FAR) 48 C.F.R. Chapter 1 Subpart 9.4. Bidders are required to certify under penalty of perjury that neither they nor any proposed subcontractors are debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. The certification must be executed by a corporate officer of the bidding company and includes the legal name of the bidder, the officer's signature, title, and printed name.
The debarment and suspension certification is a mandatory compliance requirement for all bidders responding to the CCSD Student Support Contracted Services RFP. Bidders who are unable to execute this certification will be deemed ineligible for contract award. Additionally, if at any point during the bid process or after contract award, a bidder becomes debarred or is placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors by any Federal entity, the bidder must immediately notify CCSD. All certifications will be verified by the district prior to award. The form was last updated on August 23 and represents standard pre-award compliance documentation required by CCSD for procurement activities.
