About this file

This is a Required Form—Conflict of Interest Questionnaire document associated with RFP #13-13-26 for Vending Machine Services issued by the City of Temple, Texas. The RFP seeks to establish an annual services agreement for the supply, installation, and servicing of vending machines at designated municipal facilities. Proposers must submit required forms including the Proposer Information Form (Form 5.1), Proposer Questionnaire Form (Form 5.2), and Conflict of Interest Questionnaire (Form 5.3). Submissions may be made electronically or by paper according to specified instructions. The successful vendor must execute a service agreement substantially in the form provided by the City, with no substantive changes permitted. The agreement term and specific response/due dates are referenced in the RFP timeline, though complete details are not fully visible in this document excerpt.

The awarded vendor is responsible for all costs related to vending machine services, including supply, installation, and ongoing maintenance. Monthly invoices in arrears must be itemized and submitted to invoice@templetx.gov or mailed to the City of Temple Purchasing Department, clearly indicating service locations, dates, and the City's purchase order number. The vendor must maintain insurance coverage as specified in the attached service agreement and must demonstrate ability to secure such coverage within five days of award notification. The City reserves the right to add, modify, or delete service locations or property through contract amendment. The vendor is liable for all damages to people and property resulting from contracted work, and unsatisfactory performance—including excessive service callbacks, lack of personnel expertise, or failure to provide timely service—may result in contract termination.
